Objective: To observe the blood glucose changes before and after H. pylori radical therapy and the possible mechanism.Part One The Prevalence of Hp infection in general population and the relation to the glucose metabolism.Methods: 378"normal"cases were chose from general population. Blood biochemistry, glycated hemoglobin (HbAlC) and serum Hp antibodies were determined.Results: The HbA1C levels were higher in the population of Hp infection than the negative controls(P<0.05).Part Two The changes of glucose metabolism after radical therapy of HpMethods: 35 Hp+ patients were chose from the part one population (378 cases) and were divided into 2 groups (Control group n=17 and treatment group n=18). Diet control were given to each group and the treatment group were given radical therapy for Hp infection. Each group was devided into 2 subgroups according to their blood glucose ( normal glucose regulation (NGR) and intoralant glucose regulation (IGR)).Result: FBG, 2hBG and HbA1C didn't change significantly after radical therapy in the treatment-IGR group (P>0.05) and were elevated in the Control-IGR group (P<0.05). HbA1C in the treatment group after therapy was lower than the control group(P<0.05).Part Three Relation between Hp infection and inflammatory factorsMethods: Inflammatory factors (hsCRP, IL-8,TNF-α) were detected in 32 cases, who were devided into 4 groups with 8 cases in each group (Hp-NGR group,Hp+NGR group,Hp-IGR group and Hp+IGR group)Results: Inflammatory factor levels were higher in the Hp+NGR group,Hp-IGR group and Hp+IGR group than that in the Hp-NGR group. The levels are the highest in the group of Hp+IGR.Conclusions:(1) The HbA1C level is higher in the population of Hp infection older than 45, indicate that Hp is involved in the pathogenesis of type 2 diabetes mellitus.(2) Radical therapy for Hp can reduce blood glucose and HbAlC, which may delay the progress of type 2 diabetes mellitus in this special group. (3) Inflammatory factor levels (hsCRP, IL-8, TNF-α) are higher in the Hp+ cases, which suggests that the inflammatory factors are playing important roles connecting Hp infection and diabetes.
