Objective:To investigate the related factors of prognoses which influence fulminant acute pancreatitis (FAP). Identify the risk factor from clinical data which caused the Patients death.Methods:According to the set standards,58 cases of in-patients with FAP were collected who received for therapy in Sichuan Academy of Medical Sciences & People's Hospital of Sichuan Province from January 2003 to January 2011. Cases were divided into death group and survival group according to their outcomes. We retrospectively analyzed their pathogenesis, the result of laboratory examination in 24 hours after they were hospitalized, Operations, and the scores of APACHE II, SOFA & Balthazar CT, etc. Analyzed the difference between two groups, Logistic regression analysis method was used in the related factors of death.Results:58 cases in-patients of FAP,25 cases in survival group and 33 cases in death group. The mortality rate of was 56.89%. Comparison of two groups:(1) The survival group and death group in age, gender and etiology without statistic difference (P>0.05). (2) The BUN,CR,TBIL,DBIL,WBC and HCT are obviously higher than survival group within 24 hours be hospitalized, but the ALB,CA are lower than survival group, statistic difference (P<0.05). And AMY,LPS,K,NA,BS have not statistic difference. (3) The respiratory dysfunction stands first of the list for the organs dysfunction in the two groups as 64% and 69.7%. For the numbers of organ dysfunction, single, ternate and above ternate have not significant difference in two groups (P<0.05) as 72% single and 0 ternate and above ternate organ failure in survival group, and 9% single and 57.5% ternate and above ternate organ failure in death group. Meanwhile, double organ failure has no statistic difference. (4) Operation in two groups has not statistic difference. (5) No statistic difference for the scores of APACHE II, SOFA and Balthazar CT in two groups. (6) The logistic regression analysis shown that the WBC,BUN in the beginning of being hospitalized and≥3 multiple organs dysfunction are the risk factor of FAP death.Conclusions:1,CR,HCT,ALB,TBIL,DBIL and CA in the beginning of being hospitalized have relationship with FAP death, and will contribute the determination value for the prognoses.2,WBC and BUN in the beginning of being hospitalized and more than 3 multiple organs dysfunction are the risk factor of FAP death.
