Objective To investigate the risk factors for the early death of patients with severe acute pancreatitis(SAP).Methods Clinical data of 172 patients with SAP,who were referred to our hospital from January 2008 to December 2009, were analyzed retrospectively.The patients were divided into death group 157 cases and survival group 15 cases according to their outcome. Clinical and laboratory data within 24 hours after admission of those patients were analyzed. And Logistic regression analysis was performed.Results 15 of 172 SAP patients were dead.The mortality of SAP was 8.72%. There were significant differences in breath rate,serum creatinine,BUN,serum glucose,serum natrium,serum albumin[(21.25±3.15) vs(25.80±7.28), (83.82±53.65) vs(337.23±297.9), (7.34±8.57) vs (15.81±7.73), (8.47±2.63) vs (15.87±3.4), (136.60±5.26) vs (139.64±5.68), (39.26±30.08) vs (32.59±5.88), P<0.05]. And age,heart rate,HCT,mean arterial pressure,blood and urine amylase,et al did not have significant differences between two groups.The incidence rate of MOF,respirstory and renal of the fatality group was higher than the survival group.The Multivariate Logistic regression analyss showed that te dangerous factors relating to death were MOF,respiratory.Conclusion It is useful to evaluate the prognosis in breath rate,serum creatinine,BUN,serum natrium,serum albumin.The risk factors related to early death of SAP patients include MOF,respiratory and serum glucose.
