| Objective:To study the expression of TPS,OPN and CEA in the serum from the patients suffered from colon cancer and healthy examination people,and detect expression of TPS,OPN and CEA in the peripheral blood from the patients suffered from colon cancer in different gender,different age,lymph node metastasis,different depth of invasion,and different size,and hope to reveal the function of TPS,OPN and CEA in the process of the genesis,development,infiltration and transfer of the colon cancer;We Explorried the possibility of TPS and OPN as tumour markers combined CEA for early diagnosis,and use epidemiology to evaluate the clinical value.Methods:select 84 cases of patients with colon cancer who received surgical resection at the Taian central Hospital during from December 2009 to February 2011,of the patients,44 were male and 40 were female. The median age was 59.29 years(range,40-77). All the samples were proved by pathology.In the research, the patients were strictly selected according to the request;the common group were healthy examination people(observe other indicators discharge the presence of neoplastic diseases).1. The concentration of OPN,TPS in the serum was detected by double antibody sandwich enzyme-linked immunosorbent,while the concentration of CEA were measured by electrochemical luminescence in the hospital laboratory .2. Statistical Methods: Using Excel 2003 to establish a database, all statistical analyses were performed with SPSS 17.0 and PEMS 3.1 statistical software, the cutoff value of TPS,OPN was obtained by ROC curve in the serum. Diagnostic values of these three tumor markers and combined detection were analyzed by Epidemiology. Results:1. The serum level of TPS,OPN and CEA in patients with colon cancer were higher than in the common group,and there were significant difference (P <0.05).2. The serum level of TPS,OPN and CEA in patients with colon cancer had no significant difference in different gender and different age (P>0.05).3. The serum level of TPS,OPN and CEA was significantly higher in the patients with colon cancer with lymph node metastasis than that in those patients without lymph node metastasis,and there were significant difference (P <0.05).4. The serum level of TPS,OPN and CEA was significantly higher in the patients with colon cancer with invasion into the serosa than that in those patients invasion into the muscular layer,and there were significant difference (P <0.05).5. The serum level of TPS,OPN was higher in the patients with colon cancer with tumor diameter≥5cm than that in those patients with tumor diameter <5cm,but there were no significant difference (P>0.05);The serum level of CEA was significantly higher in the patients with colon cancer with tumor diameter≥5cm than that in those patients with tumor diameter <5cm, and there were significant difference (P <0.05).6. when obtained the TPS cutoff values of 59.89 U/L, the sensitivity and specificity of the patients with colon cancer were 69.05% and 85.71%,respectively; when obtained the OPN cutoff values of 32.01μg/L, the sensitivity and specificity of the patients with colon cancer were 54.76% and 92.86%, respectively; when obtained the CEA cutoff values of 3.4ng/L, the sensitivity and specificity of the patients with colon cancer were 47.62% and 92.86%.7. TPS,OPN detection rate of colon cancer was higher than CEA, and the combined determination had more clinical value than single8. Combined determination of TPS,CEA and OPN showed the highest sensitivity (97.62%), and its specificity, Youden,Kappa,positive predictive value (PPV) and negative predictive value (NPV) were 73.81%,0.71,0.75,88.17%,93.93%, respectively.9. Use the the line×listχ2 test of association to related analytical method shows the TPS and CEA has no relativity(P>0.05);the OPN and the CEA has no relativity(P>0.05).Conclusion:1. The serum level of TPS,OPN and CEA in patients with colon cancer were higher than in the common group.2. TPS,OPN and CEA can be regarded as the serum biomarkers for the colon cancer,take part in the development of colon cancer.They may be regarded as the important biomarkers for early diagnosis.3. Serum TPS, OPN, CEA could be used as diagnostic indicators of colon cancer;the combined determination had more clinical value than single and combined determination of TPS,CEA and OPN showed the best clinical value.4. The serum TPS and CEA for colon cancer had no relativity and the OPN and CEA for colon cancer had no relativity which indicated that TPS, OPN were not substitutes for CEA as clinical indicators and they had different roles. |
