Objective: To evaluate the application value of internal intestinal splinting with long nasointestinal tube in management acute small bowel obstruction.Methods: Retrospective analysis 150 cases which are diagnosed as mechanical small bowel obstruction by abdominal plain film and MSCT, during Apr. 2005 and Dec. 2010 in the department of general surgery(3) of First Affiliated Hospital of Dalian Medical University. There are 50 cases in treatment group (internal intestinal splinting with long nasointestinal tube) and 100 cases in control group (traditional ileus surgery without internal intestinal splinting). Comparing with the postoperative exhaust time, the postoperative defecation time, the rate of postoperative complications and the postoperative mortality of these two group. Followed up for 4 months to 6 years after the operation. Results 129 cases were followed up(41 cases in treatment group and 88 cases in control group). And comparing with the postoperative recurrence rate and recurrence time in 5 years.Results: The postoperative exhaust time of the treatment group was (4.78±2.35)d, and(5.01±2.40)d in the control group,(P>0.05); The postoperative defecation time of the treatment group was (5.60±2.83)d, and(5.64±2.66)d in the control group,(P>0.05); The rate of postoperative complications was 18 %(9/50) in the treatment group, and 14% (14/100) in the control group,(P>0.05); The postoperative mortality was 2%(1/50) in the treatment group, and 3%(3/100) in the control group,(P>0.05); The postoperative recurrence rate was 4.88%(2/41) in the treatment group was obviously less than the 18.18%(16/88) in the control group. There was significant difference between the two groups, (P<0.05). And the recurrence time in the treatment group was (42.50±7.78)m, obviously less than (20.25±11.82)m in the control group. There was significant difference between the two groups, (P<0.05).Conclusion: Compared with traditional ileus surgery, there is less recurrence rate and later recurrence time in the group of internal intestinal splinting with long nasointestinal tube. It is an ideal management in treating acute small bowel obstruction, which is worth extending widely.
