Objective To describe the current status of unplanned extubation of nasointestinal tube in the intensive care unit,and analyze the factors of unplanned extubation of nasointestinal tube;and also to form Bundles of Care by reviewing a large number of literature and selecting the specific literature combined with the research results and clinical practice,and to probe into the Intervention effects after applying the Bundle cares.Methods According to the inclusive criteria,we included patients from intensive care unit during September 1,2016 to March 31,2017 of a third-grade class-A hospital.On the basis of evidence-based nursing practice combined with the research results and clinical practice,this study formulated the bundle cares of unplanned extubation of nasoduodenal tube;Applied the bundle cares to the intervention group in May 1,2017 to August 31,2017 in the hospital ICU and used the conventional nursing measures in the same ICU during September 1.2016 to March 31.2017 as the control group,compared the incidence of unplanned extubation and the rate of catheter occlusion of two groups.Results(1)Analyzed the current status of unplanned extubation of nasointestinal tube in adults who met inclusion criteria of the research object in a third-grade class-A hospital intensive care unit from September 1,2016 to March 31,2017,the main reason of the unplanned extubation of nasointestinal tube is tube blockage and slipe out.Single factor analysis showed that the nasointestinal tube occlution was related to the number of days and speed of the nutrient solution;Multivariate regression analysis of nasointestinal tube occlution was not related to the age,gender but to the number of days and speed of the nutrient solution.(2)On the basis of evidence-based nursing practice combined with the research results and clinical practice,this study formulated the bundle cares of unplanned extubation of nasoduodenal tube.(3)The incidence of unplanned extubation was 23.7%and the rate of catheter occlusion was 13.2%of the control group,the incidence of unplanned extubation was 5.4%and the rate of catheter occlusion was 2.7%of the intervention group which was treated by bundle cares and the difference was statistically significant(p<0.05).Conclusion The bundle cares can effectively reduce the incidence of unplanned extubation and the rate of catheter occlusion with corrcet appplication in the clinical practice,and ensure that enteral nutrition has been successfully carried out.
