Background: Traditionally, mature myocardial cells terminally differentiated cells, when the myocardial infarction, heart function cells lack regenerative capacity, eventually replaced by scar tissue. In recent years, some undifferentiated cells be found in the heart of human injury,and fixed the concept of the heart is terminally differentiated tissues[1]. Many studies found that after acute myocardial infarction (AMI), the myocardial cell necrosis by the ischemia and hypoxia, and in the infarct region from bone marrow stem cells with cardiac phenotype of naive cells [2], this phenomenon is considered that the bone marrow microenvironment of autologous hematopoietic stem cell homing to infarct myocardium and the surrounding area results. Although a limited number of these naive cells, but stem cells will be linked with myocardial cell transplantation for the treatment of myocardial infarction provides a new direction. Stem cells from bone marrow mobilization to peripheral blood re-homing to injured myocardium, is a very complex process [3], myocardial infarction hematopoietic microenvironment and hematopoietic stem cell migration, proliferation and differentiation are closely related, involving a number of cytokines , Inflammatory factors, adhesion molecules involved. This study is to explore the microenvironment of cytokines in the mobilization of hematopoietic stem cells, homing to the area of myocardial injury and their correlation study in the myocardial infarction.Methods: A total of 60 cases of autopsy specimens from the Department of Pathology and Forensic Medicine, Dalian Medical University between 2009-2010. 10 normal heart samples death due to brain injury or mechanical injury, the autopsy found no abnormal heart; 50 myocardial infarction samples which had coronary artery Atherosclerosis and coronary artery luminal stenosisⅡ~Ⅳ, and died within 5days after myocardial infarction. All the specimens were autopsied within 48h after death, and the left ventricular anterior, lateral, and posterior wall were drawn to prepare tissue sections. The vascular endothelial growth factor VEGF, stromal cell derived factor SDF-1 and hematopoietic stem cell surface marker CD34 in normal myocardium, myocardial infarction and the expression of the surrounding area infarction be detected by immunohistochemistry.The main outcome measure is that the expression of VEGF, SDF-1 and CD34 in different areas and their relations.Results: VEGF is not expression or weakly expressed in normal myocardium, and positive and strongly positive expression gradually increased in the surrounding area infarction and myocardial infarction. It's positive expression of the main site of myocardial cell cytoplasm and vascular endothelial cells, the expression of these three parts are different The positive rates were 30%, 88%, 100%, the difference was statistically significant P <0.05; SDF-1 was not expressed in normal myocardium, the expression of infarct zone around the area and myocardial infarction gradually increased ,the expression of the positive parts of the cytoplasm, the positive rate was 90%, 100%, the difference was statistically significant P <0.05; CD34 is not expressed or weakly expressed in normal myocardium, the positive expression in the surrounding area infarction and myocardial infarction gradually positive and strongly, the expression of positive sites of myocardial cell membrane or cytoplasm, the positive rate was 40%, 86%, 100 % , the difference was statistically significant P <0.05; The Spearman correlation statistical analysis showed: VEGF and SDF-1 expression was positively correlated (r = 0.706, p = 0.001); CD34 and VEGF expression was positively correlated (r = 0.242, p = 0.048); CD34 and SDF-1 expression Positive correlation (r = 0.0.791, p = 0.00).Conclusion: 1.VEGF expression in the infarcted myocardium was significantly higher than at the surrounding area infarction and normal myocardium (P <0.05).2.SDF-1 expression in the infarcted myocardium was significantly higher than at the surrounding area infarction and normal myocardium (P <0.05), the expression of normal heart tissue were negative.3.CD34 expression in the infarcted myocardium was significantly higher than at the surrounding area infarction and normal myocardium (P <0.05).4.VEGF, SDF-1, CD34 in myocardial infarction and surrounding area infarction was a significant correlation between the expression.
