ObjectiveIn order to learn the status of radiation protection management in all levels of radiology institutions in Xuzhou city, control the problems and difficulties in radiation protection management, investigating and analyzing the radiation protection management situation during 2009 and 2011 in all levels of radiology institutions in Xuzhou city, and try to provide a solution for further strengthening and standardizing radiation protection management in all levels of radiology institutions in Xuzhou city.MethodsInvestigation objects included all levels of radiology institutions in Xuzhou city (all levels of medical institutions). General survey was performed. In 2009-2011 consecutive years, radiation protection management, situation of radioactive equipments, radiology licenses, construction project examination, radiation-workers'qualification, management of occupational health surveillance and radiation protection status were investigated at all levels of radiology institutions in Xuzhou city. Data entry used Epidata3.0 software. After data verification, logistic checking and abnormal value judgment and treatment were performed, The survey data were analyses by SPSS 13.0 software, statistical analysis used theχ2 test or Fisher exact probability method. With P <0.05 for the statistically significant difference.Results 1.Basic information568 of all levels of radiology institutions in Xuzhou city were investigated in 2009-2011. The level three hospitals, the level two hospital, township hospitals and community health service center accounted for 0.70%, 3.70%, 57.04%, 38.56%, separately.2.Radiation protection management status at all levels of medical organizationThe overall percent of pass was rise in the indexes include the establishment of organizations, systems establishing, quality controlling, radiation protection management archives at all levels of medical institutions in Xuzhou city. After compared the qualified rates of the four kind of medical organizations on 2009-2011, the level three hospital's qualified rates overall is higher than other kinds. In 2011, for the level three hospitals, the qualified rates at the establishment of organizations, systems establishing, quality controlling, radiation protection management archives were 100%, 100%, 100%, 96.55%.3. The configuration and the use of radioactive equipments and the administrative license568 of all levels of radiology institutions in Xuzhou city were investigated in the three years from 2009 to 2011,and the number has no changed in the three years. 2009-2011, the ratio of administrative license at all levels of radiology institutions in Xuzhou city were 37.50%, 48.94%, 71.30%. The use frequency of radioactive equipments has also been increasing each year. The state testing rate of all levels radiology institutions for these three years were 25.51%, 55.46%, 71.30%.The testing rate of radiology site were 15.49%, 24.47%, 48.06% for three years, although the three indexes were low, but showed a rising trend year by year, the highest year was 2011.4. Radiology construction project examination 2009-2011, the ratio of pre-evaluation for radiology construction project were29.03%,39.62%,55.88%,the ratio of acceptance appraisal for radiology construction project were21.28%,42.31%,60.53%.the two indexes were higher in the three level and the two level hospitals, the indexes were lower in township hospitals and community health service center because of lacking of knowledge of radiation protection laws.5. The configuration of radiation workers and their qualificationsThe configuration of radiation workers of medical institutions who underwent medical radiological therapy, nuclear medicine and interventional radiology is regulated, contrary to that, the configuration of the institutions which underwent medical diagnostic X ray is irregular. At qualifications of radiation workers, the most important problem was that some institutions employed workers who lacked qualifications for the post,some didn't alter the practicing point of the practicing physician who retired from other hospitals.6. The management of occupational health surveillance of radiation workersComparing the status of occupational health surveillance of radiation workers in all levels of radiology institutions, during 2009-2011, the qualified rates of six indexes for occupational health surveillance of radiation workers, the level three hospitals'better than other kinds'of institutions. In particular, in accordance with the regulations on radiation protection, three indexes on personal dose monitoring and arrangement of the occupational health examination, the level three hospitals, have more significant advantages. In the three years, the township hospitals got the lowest pass rates for the six indexes in all levels institutions, in 2011 was 40.50%, 40.50%, 30.43%, 30.00%, 40.89%, 39.00%. 7. Status of radiation protectionThe status of radiation protection of all levels medical institutions in Xuzhou city was better during 2009-2011.no matter in the respect of protective facilities or personal protective equipment was improved. In the three years, the pass rates of radiation room were 28.21%, 30.34%, 35.55%, separately. The pass rates of protection of doors and windows were 36.60%, 39.39%, 62.32%.The rates of radiation sign and light for work state were 18.33%, 21.03%, 25.35%. The pass rates of the use of personal protective equipment were 10.04%, 13.56%, 25.35%. The four indexes showed a rising trend year by year, the highest year was 2011. The installation of mechanical-ventilation at all levels institutions in Xuzhou city, during the investigated three years, the proportions were low but little changed, followed by 4.49%, 5.81%, 6.87%. Comparing the status of radiation protection in all levels of radiology institutions, during 2009-2011, the level three hospitals'better than other kinds'of institutions.ConclusionRadiation protection should be standardized at all levels of medical institutions in Xuzhou city. The standards of radiation protection management need to be elevated, the compliance of personal protective equipment needs to be further refined and improved. The level of radiation protection was low, especially at the township and the community health service center. Some medical institutions especially the medical institutions below the level three hospitals were unaware the importance of radiation protection, the funds investment insufficiency, the investigation and punishment to violation was weak, and the radiation protection awareness of patients and clients was not enough. They were the major problems and difficulties faced by all levels of radiology institutions.
