Objective: At the present time, the pathogenesis of diabetic nephropathy is still not clear. In resent years, have paid more and more attention to the effect on the inflammatory reaction in the pathogenesis of diabetic nephropathy. The newly adipocytokines, visfatin,It have significant relationship in the inflammatory. C-reactive protein as the most sensitive inflammation markers, Its levels can react inflammation.The article will to detect the changes of serum C-reactive protein visfatin levels in type 2 diabetic patients with nephropathy(DN), and explore the significance of visfatin and CRP in the development of diabetic nephrpathy.Methods: This study included 96 patients with type 2 diabetes mellitus,All conform to the diagnostic criteria of WHO in 1999, 48 man and 49 female, mean age of 56.8±8.7 years old. These patients were divided into three groups according to the urinary albumin excretion rate(UAER) : normalalbuminuria group (A Group, UAER<20μg/min),n=35, micro-albuminuria group (UAER20-200μg/min), n=30 and macro-albuminuria (UAER>200μg/min), n=32, and 20 healthy subjects as control group, 19 man, 16 female. serum visfatin was measured by enzyme linked immunosorbent assay(ELISA),and detect the fasting blood glucose. HbA1c. TG. TC. LDL-C. HDL-C. BUN. Scr and UAER. All the statistical analysis were dealt with SPSS17.0 soft ware. Compare the differences of the groups.Results: Serum visfatin(A group=2.58±0.10μg/l; B group =31.21±1.80μg/l; C group =48.78±2.84μg/l) and C-reactive protein(A group =3.12±0.85 mg/l; B group =8.27±1.70 mg/l; C group =16.7±2.45 mg/l)levels in the diabetic patients were significantly higher than those in controls(visfatin =17.02±1.86μg/l,CRP==1.57±0.42 mg/l) (P<0.05), and increased with UAER in the three groups. Serum visfatin concentrations were significantly positively correlated with duration of DM. SBP,HbA1c,FPG,TG,TC,LDL-C,BUN,Scr and UAER levels (r=0.943; 0.699; 0.511; 0.371; 0.362; 0.398; 0.485; 0.623; 0.832; 0.89) (P<0.01), and significantly inversely correlated with HDL-C(r=-0.617), C-reactive protein concentrations were significantly positively correlated with duration of DM. SBP,HbA1c,FPG,TG,TC,LDL-C,BUN,Scr and UAER levels (r=0.777; 0.701; 0.416; 0.327; 0.307; 0.449; 0.611; 0.817; 0.893) (P<0.01), and significantly inversely correlated with HDL-C( r=-0.546), Serum visfatin were significantly positively correlated with CRP.Conclusions: 1.Type 2 diabetes patients serum visfatin and c-reactive protein levels higher than normal people , Support that have lipid metabolism disorders and inflammatory reaction in type 2 diabetes. 2. serum visfatin and c-reactive protein levels higher in diabetic nephropathy than in simple type 2 diabetes, and increase gradually with UAER, Support that they have relation ship with the development of DN.3. Serum visfatin were significantly positively correlated with CRP.Support that visfatin have relation ship with inflammation.
