Backgrounds:Maintenance hemodialysis has become the most important kidney replacement therapy to end stage renal failure patients. With the development and improvement of hemodialysis, a novel convenient technique named high-flux hemofiltration (HF) has come out. Comparing with the low-flux hemodialysis, HF reduces long-term complications to a certain extent. Common hemodialysis removes uremic toxins of small-sized molecular solutes with high efficiency. HF is much more effective in purging away the middle-sized molecular solutes than common hemodialysis, which delays the happening of dialysis-related amyloidosis, increases the clearance of parathormone and postpones the occurrence of renal osteopathy in patients taking long-term dialysis. It has been demonstrated that HF decreases the levels of triglyceride and high density lipoprotein, raises the level of low density lipoprotein. Nowadays, the shortness of disease-related knowledge and self-management brings restriction of daily life, decrease of quality of life (QOL) and generation of psychological problems to patients having continuous hemodialysis therapy. Health-care education is one of the most important parts of modern people-oriented clinical nursing work. Till now, little research was found about the health-care education on the patients with HF treatment. It is the basilic field of nursing research that finds ways to give suitable interventions to promote the self-management behavior and improve QOL on the patients with HF treatment.Object:To evaluate the effectiveness of systematic continuous health-care education and follow-up education, by assessing the levels of disease-related knowledge, self-management and laboratory test results.Materials and methods:With random digit table, the patients taking maintenance hemodialysis treatment in the dialysis center of a hospital were separated into systematic education group and general education group. Patients in systematic education group had the continuous health-care education and follow-up education, and those in general education group only had the disease-related knowledge education when taking hemodialysis. Data were collected from both groups before and after education as follow: hemodialysis-related knowledge questionnaire, self-management behavior questionnaire, the levels of serum BUN, Scr, phosphorus, ALB and triglyceride.Results: 1. Although the disease-related knowledge levels in both groups improved, there was statistically significant difference between two groups (p < 0.01); 2. After education, patients in case group have more enhancement of self-management behavior than those in control group (p < 0.01); 3. Patients in both groups had a decrease of BUN and Scr after education (p>0.05); 4. After education, patients in case group had more obvious reduction of the levels of serum phosphorus and triglyceride, and more significant increase of the ALB level. (p < 0.01)Conclusions:1. Systematic continuous health-care education effectively improves disease-related knowledge and self-management behavior in patients having HDF treatment;2. Systematic continuous health-care education efficaciously amends the serum levels of phosphorus, triglyceride and ALB;3. Comparing to the traditional way of education, systematic continuous health-care education and follow-up education could enhance disease-related knowledge and self-management ability, which provides reference pattern for the management of other chronic disease.
