ObjectiveTo optimize Health Care Toolkit of residents in Gansu, and comprehensively evaluate the performance of promoting and application so as to provide evidence-based basis for government to further optimize the implementation plan of the project and improve the health education strategy.Methods1. Delphi method was applied to determine the components of Health Care Toolkit through two rounds of expert consultation.2. The “structure-process-result” model, a series of methods including literature review, interview, questionnaire, observation, lot quality assurance sampling were used to evaluate the implementation process and effectiveness of the Health Care Toolkit for Gansu province residents.Results1. The related health departments actively provided many resources to guarantee the implementation promotion of Health Care Toolkit in Gansu Province, including organization, finance, technology and specific arrangements and regular guidance to improve the procurement, distribution and training. “Five linkage work mode” was established to ensure the project implementation. The Toolkit program achieved some social and economic benefits.2. After 2 rounds of expert consultation, a list of Health Care Toolkit components was determined, including the configuration and technical parameters of components. There were 23 alternative indicators in the first round of expert consultation. The average score of importance was 3.94, and the maximum item scored 4.78 while the minimum was 2.89. The variation coefficient was 0.16. The indicators which the score of importance was less than 4 were removed; the remaining 13 indicators were entered into the second round of consultation. In the second round of expert consultation, the score of importance was 4.45, and the maximum scored 4.74 while the minimum was 4.17. The coefficient of variation was 0.15. The average coefficient of variation index of two rounds of evaluation was close each other, which showed that the coordination degree of expert opinion was high; the consultation result was available for decision-making.3. From 2012 to 2015, the number of cumulative distribution of the Toolkit summed to 5.2 million in Gansu province. The households’ coverage rate was more than 75.6%, and more than 71.3% of the counties in Gansu were fully covered. At the same time, a large number of workers were involved in the related health education and promotion activities. About 2.4 million residents were trained, and 2.8 million people participated in health salon activities. These activities greatly enhanced the residents’ knowledge and skill of health.4. 23 counties were selected including 112 villages in 56 towns to get random checks. The implementation and mastering degrees of the Toolkit of residents were evaluated. 26.0% of residents achieved good grades who may be looked as fuglemen; 63.0% reached the required level whose skill still need to be strengthen; 11.0% were unqualified who need to be trained again. The pass rate of health care kit training were 89.0%, the rate of female(6.4%) was less than that of male(19.7%). There were significant differences in the pass rate among the residents with the degrees of education, the ways and numbers of training and the frequency of Toolkit use(P<0.01).Conclusions1. The Health Care Toolkit for Gansu Province Residents gives consideration to two hings including health education and promotion of appropriate technologies. The design of Toolkit was innovative, economical and practical. It met the requirement of residents. The Toolkit has certain innovation and local characteristics compared with domestic similar health education kit.2. By using the health care kit, the basic knowledge and skills of health literacy effectively disseminate, the residents’ health consciousness and self care ability are improved to some extent.3. The Health Care Toolkit in Gansu province is a complex and systematic project. It still faces some difficulties and problems which need to be solved urgently, in order to make further progresses.
