Objective To evaluate the clinical value of CT perfusion in the diagnosis of senile dementia. To explore the evaluation the progression of senile dementia by CT perfusion.Methods 95 patients of senile dementia(52 with AD,43 with VD) and 20 healthy subjects(control group) underwent perfusion imaging by multi-slices spiral computer. The CT perfusion images were analyzed using perfusion software .Derived perfusion parameters including cerebral blood volume(CBV),cerebral blood flow(CBF),mean transit time(MTT) and time to peak(TTP) were measured. To assess the value of perfusion parameters in the diagnosis of senile dementia by SPSS13.0.We can evaluated diagnostic value of perfusion parameters by Logistic regression and ROC curves.Results AD group and the healthy group, the right entorhinal area ,AD group 15.13±6.52, control group 20.11±5.05. right hippocampus CBF ,health group 22.08±8.80, AD group 17.32±7.23, There was statistically significant difference (P <0. 05), and perfusion parameters of the AD group was significantly reduced than in the healthy group. Perfusion parameters of AD was significantly correlated with the left frontal lobe MTT and the right temporal lobe MTT and the right basal ganglia MTT and the right hippocampus MTT by chi-square test of Logistic regression analysis, x2 = 72.103, P = 0.000, there was statistically significant. ROC curve analysis showed that left frontal MTT area under the curve ,73.9%, P = 0.001, Cut-off 4.45s, sensitivity 80%, specificity 80%, the diagnostic accuracy in the diagnosis of AD was moderate. VD group and the healthy groups, right hippocampus CBV, healthy group 2.56±0.84, VD group 1.99±0.89; right hippocampus CBF, health group 22.08±8.80, VD group 15.53±6.31, the difference was significant (P <0. 05) , perfusion was significantly reduced the VD group than the healthy group. Perfusion parameters of VD was significantly correlated with left subiculum MTT, right hippocampus CBV, the left temporal lobe MTT, left basal ganglia CBF showed a significant correlation by chi-square test of Logistic regression analysis, x2 = 72.997, P = 0.000, it was statistically significant. ROC curve analysis showed that the left subiculum MTT area under the curve 86.7%, P = 0.000, Cut-off 9.02s, sensitivity 84.2% and specificity of 80% in the diagnosis of AD on the diagnostic accuracy was moderate. AD group and the VD group: The perfusion values showed no significant statistical significanceConclusion The results showed that CT perfusion is valuables for the diagnosis of senile dementia. .
