Objective:To investigate the value and significance of high-risk human pa-pillomavir us (HR-HPV)DNA testing combined with liquid-based cytology (TCT) and Colposcopy with directed cervical biopsy in diagnosis of cervical lesi-ons.Methods:A total of 600 outpatients cases (mean age 38.25±11.12 years)had been detected by liquid·based cytology test and HR-HPV DNA testing,in which 389 cases received cervical biopsy because of abnormal cytology(≥ASCUS). Eventually, cervical hostopathologic findings is the gold standard for diagnosis. Analysis the incidence of cervical lesions in outpatient in this hospital. Then we analyze the result ofpathological. result..Results:1. In the 600 patients, whose HPV infection was 285, the rates of HPV infection were 47.5%. The HPV infection high rate age among 31-40.2. In the 600 patients, in which HPV infection was 285, rates of HPV in-fection were 24.14%(51/211) in normal and inflammatory cytology,63.2% (36/57) in ASCUS 49.09%(108/220) in LSIL 66.13%(83/123) and 93.48%(43 /46) HSIL. The rate Was I ncreased with aggravation of cervical cytology changes (P<0.01).3. Unusual cytology result were 389 cases (≥ASCUS),HPV inrection 234 c-ases, the rates of HPV infection were 23.30%(52/161)in chrenic eervicitis and tratified squamous epigthelia papillomatous hyperplasia,63.75%(51/ 80) in CIN I,82.95%(73/88) in CINII,96.00%(48/50) in CINⅢand 100% (10/10) in invasive cevical cancer. THE rate was increased with aggravation of cervical histology changes There was statistically significant difference in the positive rate between(≥CIN) and normal or inflammation(P<0.01).4. Among 220 patients with abnormal cervical cytology of ASCUS, the rate of high-grade cervical lesions wash higher in HPV positive Group 27.08%(39 /144) than in HPVnegative group 7.89%(6/76).There was statistically s-ignificant difference in the positive rate between (≥ASCUS) and normal or inflammation. It is show that intection rate of HR HPV is ascending with se- rious digree of cervical lesion (P<0.01). Among 83 patients with abnormal cervical cytology of LSIL, the rate of high-grade cervical lesions Was ev-en higher in HPV positive group44.29%(31/70) than in HPV negative group-7.69%(1/13), High risk HPV testing can help improve detection rate of h-igh-grade cervical lesions in patients with cytology of ASC-US and LSIL. Th-ere was statistically signiricant difference (P<0.05).Conclusion:The peak incidence of cervical lesions ages of 31-40 years old, occurred in the reproductive and sexually active women. The rate was inc-reased with aggravation of cervical histology changes. Higll-risk HPVDNA testing can improve detection rate of high-grade cervical lesions in pati-ents with cytology of ASCUS and LSIL and prevent., misdiagnosis and impro-per therapy. The combination of high-risk HPV testing, liquid-based cytolo-gy and colposcopic biopsy can greatly improve the rate of cervical lesions.
