Objective:To investigate the clinical implications of procalcition(PCT) in patients with Adult-Community acquired pulmonary(CAP).Material and Methods:Material:Sixty-six patients with newly definite diagnosed of community acquired pulmonary(CAP) admitted into Shandong Provincial Hospital from January 2010 to December 2010 and 36 healthy people were enrolled into our study. CAP group included 42 males and 24 females, average age 57.2 years old(ranged 18~88 years).19/66 patients in CAP group with systemic inflammatory response syndrome(SIRS) and 47/66 patients in CAP group without SIRS. Healthy group included 22 males and 14 females, average age 58.8 years old (ranged 20~80 years).The diagnosis of CAP is according to Guidelines for Community-Acquired Pneumonia and the diagnosis of SIRS is according to the classification of APCC /SCCM in 1992.Methods:Fasting peripheral venous blood were taken in the first morning after admission to hospital. The concentrations of PCT were measured by enzyme-Linked immunosorbent assay(ELISA).Healthy people were given blood routine and erythrocyte sedimentation.rate examination.①comparing the serum level of PCT between the general CAP group and the healthy group.②comparing the serum level of PCT between non-SIRS group of CAP and the healthy group.③comparing the serum level of PCT between SIRS group of CAP and the healthy group.④comparing the serum level of PCT between the SIRS group of CAP and non-SIRS group of CAP.⑤.comparing the correlation of PCT and WBC,NEUT%,ESR between SIRS group of CAP and non-SIRS group of CAP respectively.Results:1.The serum concentration of PCT in general CAP group was (0.55±0.54) ng/ml, and the serum level of PCT in the healthy group is(0.16±0.08) ng/ml. Comparing the serum level with the two group,they have statistical significance (P<0.001).2. The serum concentration of PCT in non-SIRS group of CAP was (0.39±0.29) ng/ml, comparing to the serum level of PCT in the healthy group, it has statistical significance (P<0.001).3. The serum concentration of PCT in SIRS group of CAP was (0.92±0.80) ng/ml, comparing to the serum level of PCT in the healthy group, it has statistical significance (P<0.001).4. The serum concentration of PCT in SIRS group of CAP was (0.92±0.80) ng/ml and the the serum concentration of PCT in SIRS group of CAP was(0.39±0.29) ng/ml. Comparing the serum level with the two groups, they have statistical significance (P<0.001). Bur the level of WBC,NEUT%,ESR between SIRS group and non-SIRS group had no statistic significance, P were 0.198,0.519,0.137 separately.5. The line correlation coefficient (r)of PCT and WB,NEUT%,ESR are 0.325 (P=0.026),0.372 (P=0.010),0.030(P=0.842) respectively in the non-SIRS group of CAP while the r of the SIRS group of CAP are 0.07(P= 0.774),0.098 (P=0.688), 0.064(P=0.295).Conclusion:1. Serum PCT could be use as a marker to judge the severity of CAP. It can provide something help for guiding the clinical care.2. The line correlation coefficient (r)of PCT and WB,NEUT%,ESR are 0.325,0.372,0.030 respectively in the non-SIRS group of CAP while the r of the SIRS group of CAP are 0.07,0.098,0.064.Therefor WBC and NEUT% can be used as markers in the diagnosis of pure CAP, but they can not be used as markers to judge the severity of CAP.3. In the early period of CAP, most of the patients have obvious clinical symptoms and imaging findings, but the serum level of PCT in these patients does not have obvious rising, so PCT should not be used as a marker in the diagnosis of diagnosis of early CAP.
