The Treatment Of Unstable Femoral Intertrochanteric Fracfure With Dynamic Hip Screw And Chinese Medicine

Objective:To compare the clinical effect of femoral calcar and lesser trochanter reduced and fixed or not in the treatment of unstable femoral intertrochanteric fracture with DHS in order to offer basis on choosing the proper treatment method in clinic.Method:To choose 60 cases of unstable femoral intertrochanteric fracture treated in Harbin GuShang hospital from March 2009 to March 2010.They all tallied with the criterion that should be subsumed or eliminated.We divided them into two groups randomly with the random number table.Then one group of 30 cases wastreated with DHS and Chinese medicine adding femoral calcar and lesser trochanter reduced and fixed, the other with DHS only.To compare and analyse the clinical effect of two different methods in thecommon conditions, operative procedure, the complications and the functional evaluation of hip join.The statistical analysis was performed by SPSS13.0.Result:All the patients were followed.There were no significant difference between two groups in the neck shaft angle during operation, the volume of drainage after operation (P>0.05). The operation time were 119.40±1.42 minutes for the group of femoral calcar and lesser trochanter reduced and fixed,80.32±7.05minutes for the group of femoral calcar and lesser trochanter not reduced and fixed.There were significant difference between two groups(P<0.05). The blood loss during operation were 419.65±96.54milliliters for the group of femoral calcar and lesser trochanter reduced and fixed,348.0±80.33milliliters for the group of femoral calcar and lesser trochanter not reduced and fixed. There were significant difference between two groups(P<0.05). The number of perspective times during operation were 9.33±1.42times for the group of femoral calcar and lesser trochanter reduced and fixed, 5.90±1.79times for the group of femoral calcar and lesser trochanter not reduced and fixed. There were significant difference between twoAccording to the Harris'S hip functional standard score, the scores were 90.32+2.58for the group of femoral calcar and lesser trochanter reduced and fixed,82.05±4.61 for the group of femoral calcar and lesser trochanter not reduced and fixed.There were significant difference between two groups(P<0.05). Conclusion:1,There were no statistical difference in two groups in the volume of blood transfusion during operaion.the volume of drainage after operation and the complications.2,The group of femoral calcar and lesser trochanter reduced and fixed is longer in the operation time, more in the blood loss during operation and better in the function of hip joint.3,It is very important to reduce and fix femoral calcar and lesser trochanter for unstable femoral intertrochanteric fracture treated with DHS.
