Abstract/Summary:
AIDS is the human immunodeficiency virus invading the human body caused by a high mortality rate of serious infectious diseases. AIDS anti-retroviral drug treatment can maximize the application of inhibition of viral replication, so that the damaged immune function recovery, thereby delaying disease progression, life prolong and improve quality of life. Chinese State Council put forward AIDS prevention and control of the "Four Frees and One Care" policy measures in 2003, policies which provide an element of AIDS antiviral drugs for free. Anti-viral treatment of AIDS to evaluate the anti-viral therapy of patients can understand the living conditions and survival after in order to further standardize the anti-viral treatment, reduces mortality in patients receiving treatment and improve treatment.ObjectiveTo understand and evaluate the therapeutic efficacy and 5 years'survival rates of anti-retroviral therapy in 81 adult HIV/AIDS patients. To provide a scientific basis for reducing the mortality, improving the therapeutic effect and further regulating the Anti-retroviral therapy.MethodsFrom June 2003 to December 2006,81 AIDS patients commenced antiretroviral therapy, and were followed to December 2010. Regular monitoring CD4+T lymphocyte changes during follow-up, plasma viral load testing began in 2007.The death and cease cases were recorded. Then found out the causes and date. If apparent side effects or virological failure appears, we would change the regimen.ResultsAmong 81 patients,18 patients died during follow-up. In whom,16 died from AIDS related diseases,2 died of suicide.5 patients withdrew from treatment due to compliance problems out in 3 cases and 2 cases of side effects. Because of treatment failure,3 patients had changed for second-line treatment. Average treatment time of first-line treatment was 61 months. About 63 cases (77.78%) patients developed various side effects. A few patients also appeared more than 2 or 2 toxicity.56 patients were followed-up for more than 5 years. After 5 years'treatment, the average CD4+T lymphocyte counts had reached 422/ul, increased 283/ul than baseline.5 years after treatment,32.61%(15/46) of patients fully recover immune reconstitution, 10.87%(5/46) in patients with immune reconstitution is not ideal.64%of the cases had HIV RNA levels<50 copies/ml. The Probability of death was 12.10%after 1 year and 5 year survival rate was 79.16%. The survival rates of 1,3 and 5-year were 88.50%(95%confidence interval,82.22%,94.78%),81.89%(95%confidence interval,73.03%,90.75%) and 79.16%(95%confidence interval,69.80%,88.52%).ConclusionsNational implementation of the free anti-retroviral therapy had received a better therapeutic efficacy. During treatment CD4+T lymphocyte count continued to rise, the immune function was restored to different degrees. Some patients had obtained complete immune reconstitution after 5-year's treatment.64%of the cases had HIV RNA levels< 50 copies/ml. The survival rate had changed rapid in the first year, declining to reach steady state after 2-5 years. The first year of treatment is a critical period. Comprehensive and standard treatment should be provided. Various factors that may endanger the life should be process timely. That can contribute a lot to lower mortality and better therapeutic effect.
