Objective To explore the basic situation of injuries of Zhejiang city and rural community residents; to analyze the risk factors of injury, to determine the focus of injury control groups which provide the basis for the development of injury prevention and control plans and strategies.Methods 10 counties (cities, districts) and a total of 12,000 families which include all of the resident population in the household (living more than 6 months) were collected for the survey using multistage stratified cluster random sampling method. Uniform trained investigators used questionnaires to understand injuries over the past 1 year, related factors and the basic situation of the family. After passing the examination, Epidata3.1 software was used for data entry, SAS9.13 and Excel2003 for statistical analysis. The rate and composition of the index were mainly used to describe the index,1% of 2005 National Population Sample Survey data was used for standardization.Results A total of 35,242 people, of whom 17,727 were men,17,515 were women, male to female ratio 1.01:1.20240 were city residents (male 10131, female 10109 people), accounting for 57.43%, rural 15002 (male 7596, female 7406 people), accounting for 42.57%. The average age is 42.06 years with the minimum 0 years, maximum 101 years and a median of 43 years. The age structure of the sample population is different from 1% of the population in 2005 sample survey data with a lower proportion of low-age group. Education level in Urban is higher than in rural (χ2> 1000, P<0.01), males higher than females (χ2= 79.59, P<0.01). Adults over the age of 18, often drinking accounted for 17.98%, regular smokers accounted for 21.47%.Total of 2033 cases of injuries occurred in 2002 people, injury rate was 5.77%; male injury 1160 cases, female injury 874 cases, male incidence of injuries was 6.54%(4.17% urban,9.72% rural), female injury rate was 4.99%(City 3.59%,6.90% in rural areas); males injury rate was higher than females (χ2= 35.24, P<0.01). The incidence rate of 3.88% in city,8.33% in rural areas, incidence rate was higher in rural areas than in urban (χ2= 309.15, P<0.01).With 1% of the population in 2005 sample survey data, standardized adjusted rate of injury was 5.21%, 95%CI (4.98%,5.44%); male 5.98%,95%CI (5.73%,6.23%); female 4.43%,95% CI (4.12%,4.54%). With census data of Zhejiang Province in 2000, adjusted incidence of injuries was 4.99%,95% CI (4.76%,5.22%); male 5.81%,95%CI (5.57%,6.05%); female 4.19%,95% CI (3.98%,4.40%).The incidence rate differs by the age, level of education and occupation. The incidence of injuries increased with age (χ2=160.21, P<0.01), education level (χ2= 239.90, P<0.01). The incidence of injuries were different by education (χ2=261.19, P<0.01), with the incidence of injury highest in agriculture, animal husbandry and water industry (8.74%,5.59% urban,10.34% rural).The following six types of injuries ranked the top six:falls/fall (2.23%), traffic accidents (1.27%), animal injury (0.80%), sharp injury (0.70%), blunt injury (0.46%), burns (0.21%). After standardization, falls/fall (1.91%), traffic accidents (1.17%), animal injury (0.78%), sharp injury (0.64%), blunt injury (0.42%), burns (0.21%).Main injured part to the body were lower limbs, upper limbs and trunk, accounting for 39.52%, 31.69% and 11.81%, respectively; after injury, self-treatment, outpatient, emergency, hospitalization rates were 20.23%,53.59%,9.01% and 15.40%, respectively.Of 2033 cases of injuries,55.71% were minor injuries,31.05% moderate and severe 13.24%. Injury deaths were 13 cases (male 10 cases,3 females),54 cases were caused disability (30 males,24 female),196 cases were functional impaired (male 97 cases, female 99 cases), accounting for 0.64%,2.66% and 9.65% of the total number of injuries.The injury led to a total of 56,427 days rest, with a maximum of 395 days, minimum 0 days, median 7 days, and an average of 28.97 days; total medical expenses for the injury were 5.6231 million RMB; single cases of injury led to the maximum 200 thousand medical expenses, minimum 0 RMB, median 700 RMB, average of 2818.61 RMB. The number of days off caused by different types of damage varied greatly (Z= 364.16, P<0.01), medical expenses were also different (Z= 291.31, P<0.01). Traffic injuries and falls caused the heaviest economic and social burden.Risk factors analysis showed that gender, age, education level and drinking alcohol are influence factors for injury. Men are more likely to be injured than women; older people are morel likely to be injured than younger people; education is a protective factor, with the increased level of education, injury rate reduced; alcohol is a risk factor; smoking has no influence on injury.Conclusion There is a high incidence of injury among community residents in Zhejiang province, which causes great burden for individuals, families, society. Fall/falls, traffic injuries, blunt trauma are the focus for prevention and control in Zhejiang province, meanwhile in rural areas animal injuries (especially dogs injury) and drowning/suffocation prevention and control work should not be overlooked. Falls/fall prevention should focus on the elderly population; traffic injuries, blunt injury prevention should focus on the labor groups; animal injury prevention should focus on personnel engaged in outdoor work in the rural areas; burn injury prevention should focus on children aged 0-4.
