The Clinical Application Of Artificial Total Hip Arthroplasty In The Treatment Of Secondary Osteoarthritis To Developmental Dysplasia Of The Hip

Objective:To evaluate total hip arthroplasty in the treatment of secondary osteoarthritis to developmental dysplasia of the hip in the methods, efficacy and safety. Methods:From August 2010 to March 2011,we treated 36 hip joints in 28 patiets with secondary osteoarthritis to developmental dysplasia of the hip in the department of orthopadics. There are 7 males(9 hips) and 21 females(27 hips) with an average age of 59.3 years(rang from20 to 77 years).According to the classification of Crowe:there are 11 cases (14 hips) in typeⅠ,14 cases (17 hips) in typeⅡ,2 cases (3 hips) in typeⅢ,1 case(2 hips) in typeⅣ. Results:All 28 patients were followed up for 6 to 18 months, an average of 12.4 months, the average Harris hip score improved from preoperation average 38.82 to postopration average 90.46, and treatment has improved significantly. No postoprative wound infection or other complications such as, dislocation, femoral fracture, femoral nerve and sciatic nerve damage. All patiets postopration can get a pain-free joint, the stability of their hip joints was increased They can walk independently without the help of cruthes, leg deformities greatly improve or even disappear. X ray showed acetabulum and femoral stem prosthesis position were satisfactory, no signs of loosening and infection, healing of acetabular bone graft well. Comprehensive and assessment:excellent in 19 cases 24 hips, good in 8 cases of 9 hips,1 hip in 1 patient, good rate of 96.4%. Conclusion: Total hip arthroplasty is a good method for the treatment of secondary osteoarthritis to developmental dysplasia of the hip, and adequate preoperative preparation,learn more about the type of acetabular defect, effective soft tissue release in the opration,the maximum restore the anatomy of hips and physiological function is good foundation to rebuild and stabilize acetabular acetabular prosthesis, skilled surgical technique, postoperative rehabilitation and improvement of training is the key to successful treatment.
