Effect Of Total Hip Arthroplasty With S-ROM Prosthesis And Anatomic Acetabular Reconstruction For Crowe ? Developmental Dysplasia Of Hip

Objective:The purpose of this retrospective study was to analyze the clinical functional and radiographic results of THA with S-ROM prosthesis and anatomic acetabular reconstruction for Crowe ?developmental dysplasia of hip(DDH).Methods:Between March 2007 and February 2017,36 patients(41 hips)were selected for a retrospective study in The First Hospital of Jilin University.All patients were treated by THA with S-ROM prosthesis and anatomic acetabular reconstruction.The preoperative and postoperative differences of Harris hip score and VAS was to assess clinical results.The preoperative and postoperative imaging data was to evaluate the position of the prosthesis,the loosening,osteolysis,the osteotomy and bone grafting healing.Measuring the leg length discrepancy.Complications were recorded.Results:All patients were followed up for an average of 51 months(12-131 months).One year after surgery,Harris hip score improvedfrom36.56±9.59 points to 84.38±7.11 points;VAS improved from7.96±0.48 points to 0.58±0.82 points;The mean leg length discrepancy reduced from 3.52±1.19 cm to 0.71±0.93 cm.All the difference were statistically significant(P<0.05).All prosthesis were in good position,no loosening was found.Acetabular and the greater trochanter osteolysis were found only in one case because of the severe wear of the polyethylene liner.All osteotomies were healed well and the average time in radiographic union was 5.7 months(3-12 months).During the follow-up period,there were no significant complications such as joint dislocation and postoperative infection.2 cases had periprosthetic fractures,4 cases had numbness in the exterior of lower leg,and 3 cases had postoperative anterior patella pain.Conclusions:1.It is effective for Crowe ? DDH treated by THA with S-ROM prosthesis and anatomic acetabular reconstruction.2.Total hip arthroplasty with S-ROM prosthesis can provide flexibility and multiple selectivity for operation.But the operative process is more complicated than that of the non-modular prosthesis,which requires rich experience of total hip replacement.
