Background and Objective: Whilst the social progress and the development of transportation improve, the general incidence of traumatic brain injury (TBI) has been significantly increased, which also has been the primary cause of death in young people of developed countries. The treatment of serious craniocerebral trauma has been the most difficult clinical management, which follows unfavorable prognosis, high mortality and incident of disability. Neurosurgeons therefore emphasis on its comprehensive treatment in term of improving outcome and reducing its mortality. This article is to review the treatments of 106 cases of patients who suffered the most serious craniocerebral trauma (GCS 3') between Febuary,2006 and Autumn, 2008 in Department of Neurosurgery, Lanzhou general hospital, to which analyze and summarize appropriate clinical management. Methods: Review and analyze the clinical treatment and prognosis of 106 patients with GCS score of 3 points in severe TBI retrospectively. Results: There are 45 patients survive, of which 13 patients recovered well and the residual 10 cases, 15 cases survive with severe disability, 7 cases survive in a persistent vegetative state, rest of 61 patients died. Conclusion: The emphasis of comprehensive treatment for patients who suffered TBI with GCS 3 points are reducing hospitalization time since injury, providing decompressive hemicraniectomy and eliminating the intracranial hematoma in early stage as soon as possible, where necessary. Mild hypothermia treatment also helps to improve the prognosis and reduce TBI complications.
