Objectives:To investigate the correlation between cardiac impedance method hemodynamic monitoring and plasma BNP levels on patients with acute myocardial infarction, and to understand the early changes of cardiac function for AMI patients, so that we can further illustrate the clinical significance of cardiac impedance method hemodynamic monitoring and plasma BNP levels on patients with AMI.Methods:All 30 cases of patients with AMI are from emergency intensive care unit of the first people hospital of Jining, including male 21 cases, female 9 cases. All patients were confirmed AMI through emergency coronary angiography.Then, another 10 cases were recruited as control group, including male 8 cases, women 2 cases,all of them are identified as a healthy persons through physical examination, ECG examination, laboratory examination, coronary angiography and other related inspection. History acquisition:patient sex,age, height, weight, and heart rate, systolic blood pressure,diastolic blood pressure, body mass index and body surface area. Extraction venous blood:plasma BNP levels were detected through SOB detection board provided by BIOSITE company; Central venous pressure(CVP) and the various cardiac impedance parameters were monitoring by Beneview T5 guardian instrument: stroke volume(SV), stroke index (SI),cardiac output (CO),cardiac index (CI), left ventricular stroke work(LVSW),left ventricular stroke work index (LVSWI), left cardiac ventricular work(LCW),left cardiac ventricular work index(LCWI), striction time ratio(STR), pre-ejection period(PEP),left ventricular ejection time(LVET), velocity index (VI), acceleration index (ACI), thorax fluid capacity(TFC), thorax fluid index(TFI), systemic vascular resistance(SVR),systemic vascular resistance index (SVRI),pulmonary vascular resistance(PVR), pulmonary vascular resistance index (PVRI); cardiac ultrasound EF values were recorded. SPSS 11.0 statistical software was adopted for statistical analysis. Each patient of general data, cardiac impedance and ultrasonic EF value, blood of BNP for data to mean±standard deviation.Each group do t-test for the differences. Plasma BNP levels and cardiac impedance hemodynamic parameters were analysed by Pearson correlation analysis between the related inspection. P< 0.05 was statistically significant.Results:Plasma BNP levels in AMI group were higher than that of control group, there is significant statistically difference of BNP levels between the two groups. The values of EF and CI in AMI group were significantly lower than that of control group,(both P< 0.01).EF values were positively correlated with the CI values. BNP levels were markedly negative correlated with the parameters of SI, SV, CO, CI reflecting left ventricular pumping function,and were observably negative correlated with LVSW, LVSWI, LCW, LCWI, VI,ACI parameters reflecting left ventricular systolic function,and were observably positive correlated with PEP and STR,and were negative correlated with VI,ACI, and were unrelated with TFC,VEPT, PVR,PVRI, and positive correlated with SVR,SVRI, and positive correlated with TFC,otherwise negative correlated with TFI.Conclusions:There is positive correlation between BNP levels and cardiac impedance main parameters in patients with AMI. Plasma BNP determination and cardiac impedance method hemodynamic monitoring can be used for judging early cardiac function in patients with AMI.
