Research background:With the development of social economy in rural areas and changes in structural condition of the residents, the rural resident health service utilization has undergone profound changes. After the implementation and improvement of the new rural cooperative medical system, the state is pushing forward the reform of public hospitals, and the rural health work is being faced with new problems and challenges. To promote rural health reform and development, Dongying Municipal Health Bureau and the Social Medicine and Health Management Research Institute, Shandong University jointly conducted this survey of health service utilization.Research Purpose:This is one of the rural health service utilization research series, the aim of the research is to understand the rural residents demographic characteristics in Dongying City and the influencing factors of rural health service utilization, to put forward recommendations for improving health service utilization of rural residents and the level of medical care.Research methods:In this study, stratified random sampling method was used, in accordance with the questionnaire survey of all residents in different economic levels of village or neighborhood. The survey include:demographic characteristics, medical expenses means, the prevalence of diseases in two weeks, the diagnosis and treatment of illness in two weeks and out-patient medical consultation satisfaction. The statistical data was processed by professionals with SPSS17.0 statistical analysis software. Main results:Through the sample analysis and Logistic regression model analysis, we have the results as following:1. the average prevalence rate for Dongying City residents in two weeks is 19.20%, and 48.20%for the elderly over 65 years old.2. The average attendance rate in two weeks is 31.03%, and it is higher in higher education group and people with medical insurance.3.55.88%of patients choose self-treatment of the illness. 4.89.77%of patients choose local health institutions for treatment, and 64.10%of patients choose the village health clinic or community health service stations.5. The main reasons for not seeing the doctors are mild illness, no effective treatment measures and economic difficulties, which accounted for 45.51%,26.40%and 24.16% respectively.6. The patient satisfaction rate for outpatient services was 95.00%, the main aspects of dissatisfaction are due to poor technology and high medical expenses, accounted for 31.58%respectively.7. the most important factors affecting rural health service utilization are the perceived disease severity and when the disease occurs.Conclusion:the important reasons for increasing demand for out-patient health services are the aging population and delayed healing for chronic diseases in rural residential area in Dongying City; The new rural cooperative medical care system has become the main way of health insurance for rural residents; The major factors affecting health service utilization are perceived severity of illness, when the disease occur, age, education level and participation in medical insurance; The main factors affecting the out-patient service satisfaction rate are poor technology and higher medical costs; local health system is the primary medical institutions for rural residents, and strengthening the local primary health organizations construction is the effective way to solve the problems of expensive medical care and difficulty medical treatment of residents.Countermeasures and suggestions:To establish and improve the rural elderly out-patient service system for the high demand of people; To adhere to the prevention and treatment simultaneously, and establish and improve screening and prevention system of chronic disease; increase funding levels, give full play to the new rural cooperative medical insurance as the leading role; based on grass-roots organizations, to continuously improve the level of disease diagnosis and treatment and the quality of medical services; strengthen health education, and improve the rural residents awareness of common diseases and frequently occurring diseases, strengthen government guidance, and control medical costs and treatment costs through a number of ways; adhere to complementary advantages with each other to protect non-state medical institution for medical safety and quality.
