Objective:To preliminary establish the pharmacometabonomics diagnostic models for hypertension due to hyperactivity of liver fire syndrome, dampness and phlegm syndrome and deficiency of yin and hyperactivity of yang syndrome. Some metabolites which would be promising indicators for sensitivity of Nifedipine Controlled-release Tablets (Adalat Bayer) and felodipine sustaind-release tablets (Plendil AstraZeneca) were screened,and then exploring individualized therapy of hypertension.Methods:This study selects 64 patients(age54.48±8.66, height 1.73±0.06, weight71.78±8.59) with essential hypertension as subject and 25 healthy (age50.84±8.73, height 1.74±0.06, weight67.68±9.49) as control group. The two groups did't have statistical significance in the following aspects:the age, the sex, the body height and the body weight index.They are comparable(p<0.05).The selection is made under the Case Selection Criteria. The participants are chosen from Department of Cardiology in Jiangsu Provincial Hospital of TCM and the physical examination center from 2010.3 to 2010.9. The clinical indexes and biochemical parameters of each groups of participants are observed. Because of a wide spectrum of plasma metabonomics analysis which organized on the blood sample of patients with healthy people and hypertensive people, biological markers related to hypertension and syndromes and sensitivity can be obtained.Results:(1) The population of groups in Hyertension, hyperactivity of liver fire syndrome, dampness and phlegm syndrome and deficiency of yin and hyperactivity of yang syndrome significantly high then the control group in BMI,TG,TC,HDL-C,LDL-C,Glu,UA,BUN, there were statistically significant(p<0.05).(2) The patients of hyertension, hyperactivity of liver fire syndrome, dampness and phlegm syndrome, deficiency of yin and hyperactivity of yang syndrome and healthy population have differences in metabolic spectra.(3) The biological markers related to sensitivity of Adalat are Threonic acid, Glyceric acid, Glycine, Urea, Phosphoric acid and Oleamide. The biological markers related to sensitivity of Adalat are Glucose, Omithine and Urea. Conclusions:(1)Hyertension is not only abnormality in hemodynamics, but also in metabolism,such as blood glucose, blood fat and uric acid. (2) The use of pharmacometabonomics methods can successfully distinguish between hypertensive people, hyperactivity of liver fire syndrome, dampness and phlegm syndrome, deficiency of yin and hyperactivity of yang syndrome and control group.(3)The distinguished model of hypertension which used the pharmacometabonomics technique might be helpful to build the technic platform of sensitivity of antihypertensive agent and provid thread of individualized therapy.
