Objective:To investigate the mechanism of action on preventing circum-prothesis osteolysis in artificial joint with total flavonoids of drynaria and alendronate and provide rationale clinical research and applications.Methods:50 adult New Zealand rabbits were randomly divided into negative contrasting group (group A), positive contrasting group (group B), total flavonoids of drynaria group (group C), alendronate group (group D) and combination group (group E). Two titanium Kirschner wire were inserted in knee of the rabbits' RT. hind legs by surgery for simulating total knee prosthesis. After two weeks, suspension UHMWP particles was injected into the knee of the rabbits' operated side of group B, group C, group D and group E and group A was injected with physiological saline. In B, C groups, total flavonoids of drynaria had been respectively perfused into the empty stomach daily for 16weeks.In D, E groups, alendronate had been respectively perfused into the empty stomach daily for 16weeks. The blood was collected by ear vein of each rabbit (6~8ml) at the 22th week for the blood serum Ca2+, P concentration detection. Periprosthetic bone mineral density was scanned by DEXA and it was analyzed. The collected synovia was detected the level of IL-1 and TNF-ain ELISA. The rabbits were sacrificed and obtained soft tissue around the prosthesis stained with HE histological observation. Sections of proximal tibia were counted each region tartrate-resistant acid phosphatase positive cells. All data were statistically analyzed by SPSS 13.0 software.Results:1. The blood serum Ca+ concentration of combination group (group E) compared with positive contrasting group (group B), total flavonoids of drynaria group (group C), alendronate group (group D) was statistically significant (p<0.05), and lower than the three groups.2. Positive contrasting group (group B), alendronate group (group D) was insignificant between the blood serum P concentrations (p<0.05). Total flavonoids of drynaria group (group C), combination group (group E) is insignificant between the blood serum P concentrations (p>0.05).3. The level of IL-1 of combination group (group E) in the synovia was statistically significant compared with positive contrasting group (group B), total flavonoids of drynaria group (group C), alendronate group (group D) (p<0.05), and lower than the three groups.4. The level of TNF-a of combination group (group E) in the synovia was statistically significant compared with positive contrasting group (group B), total flavonoids of drynaria group (group C), alendronate group (group D) (p<0.05), and lower than the three groups.5. Periprosthetic bone mineral density of combination group (group E) was statistically significant compared with positive contrasting group (group B), total flavonoids of drynaria group (group C), alendronate group (group D) (p<0.05), and higher than the three groups. 6. The number of tartrate-resistant acid phosphatase positive cells was statistically significant compared with positive contrasting group (group B), total flavonoids of drynaria group (group C), alendronate group (group D) (p<0.05), and lower than the three groups.Conclusion:Combined application of Total flavonoids of drynaria and alendronate could effectively inhibit the level of IL-1 and TNF-αwhich resulted from abrasions particles in the synoia; combined application could adjust the blood serum Ca2+, P concentration of model Rabbit; it significantly increased the value of periprosthetic bone mineral density; it could effectively inhibit osteolysis by osteoclasts. Experimental research shows total flavonoids of drynaria and alendronate combination could be more effective inhibition of circum-prothesis osteolysis in artificial joint.
