Objective:Investigating possibility of obviating circum-prothesis osteolysis in artificial joint of model-rabbits with total flavonoids of drynariae. To provid rationale clinical research and applications what drynaria of traditional Chinese medicine obviate prosthesis loosening of artificial joint.Methods:50 adult New Zealand white rabbits were randomly divided into blank control group (I group), abrasions particles group (Ⅱgroup), low-dose treatment group (Ⅲgroup), medium-dose treatment group (IVgroup) and high-dose treatment group (Ⅴgroup). Two titanium Kirschner wire will be insertion in aticular genu of the rabbits's RT. hind legs by Surgery for simulating total knee prosthesis. After 4 weeks of Surgery, calcium phosphate cement particles and ultra-high molecular polyethylene particles will be injected in aticular genu of groupⅡ,groupⅢ,groupIVand groupⅤ, group I was injected with physiological saline. GroupⅢ,ⅣandⅤwere respectively injected with total flavonoids of drynariae every other day with 0.3ml,0.5ml,0.7ml(lmg/ml), the other two groups do not give any medication. all the free food. After 24 weeks rabbits were sacrificed, flushed the articular cavity with 10ml physiological saline, collected washing liquid in order to centrifuge for 10 minutes with 5000r/s speed, gathered supernatant liquid and filled in Eppendrof tube, preserved it in deep-freeze equipment at-25℃, detected the levels of IL-1,IL-6 and TNF. Dislodged femur and insert,taken orthotopic roentgenogram at identical condition, X-ray films will be imported computer image analytical system, to analyse the numerous of bone density with 1.5mm thickness of the circa-insert.(3)Taken the upper tibia for decalcified sections, observed under light microscope for pathological changes around the prosthesis after HE staining.Effect:The levels of TRACP in Wear particles Group(groupⅡ) is higher than eachother,and the defferent is increases significantly(P<0.05). TRACP levels in each Treatment groups are significantly lower than Wear particles group(P<0.05), TRACP levels in each Treatment groups distinction is not statistically significant(P>0.05). The levels of ALP in Wear particles Group(groupⅡ) is higher than eachother,and the defferent is increases significantly(P<0.05). ALP levels in each Treatment groups are significantly lower than Blank control group (groupⅠ), Compared with Low-dose treatment group, ALP levels in meta-dose treatment group and High-dose treatment group is higher(P<0.05).Compared with Blank control group (groupⅠ), contents of each inflammatory factors in Wear particles Group(groupⅡ) increases significantly(P<0.05). IL-6 levels in each Treatment groups are significantly lower than Wear particles group(P<0.05),Compared with Low-dose treatment group and meta-dose treatment group,IL-6 levels in High-dose treatment group is lower,but distinction is not statistically significant(P>0.05). IL-1 levels in each Treatment groups are significantly lower than Wear particles group(P<0.05), IL-1 levels in the low-dose treatment group is the highest, followed by medium-dose treatment group, high-dose treatment group is the lowest, and the difference between the three groups is statistically significance (P>0.05). TNF-a levels in each Treatment groups are significantly lower than the Wear particles group(P<0.05),Compared with Low-dose treatment group and meta-dose treatment group,TNF-a levels in High-dose treatment group is lower obviously(P<0.05). Value of BMD in Blank control group is highest,and is significant difference from each other groups(P<0.05), Value of BMD in Wear particles group is lowest(P<0.05), Value of BMD in High-dose treatment group is highest at three Treatmentgroups,and has Statistically significant(P<0.05), Value of BMD in medium-dose treatment group is higher than Low-dose treatment group,but has not Statistically significant(P>0.05).Conclusion:Total flavonoids of drynariae can obviously restrain the level of TRACP and ALP in peripheral blood, degrad the level of IL-1,IL-6,TNF-a which were aroused by in synovia abrasions particles, restrain osteoclast activity, promote osteoblast activity, augment density numerous of circum-prothesis osseous tissue。It can prevented osteolysis which originated by wear particles after prosthetic replacement of joint, therefore Total flavonoids of drynariae may be an effective preventive drug to prevented artificial joint sterility loosen in future.
