ObjectiveThe hypertension is a cardiovascular disease, in which the systolic blood pressure (SBP) and/or diastolic blood pressure (DBP), is elevated. Moreover, it also has the most constant incidence rate. It does not only have a strong impact on the quality of life and health of human beings, but also causes serious damage to important organs of human beings, including heart, brain and kidney. Report on Cardiovascular Diseases in China (2006) indicates that there were 200 million hypertension patents in China,2 hypertension patients in every 10 adults. It has become an important public health problem in our country and thus become a major risk factor of morbidity and death caused by cerebral apoplexy, heart diseases and kidney diseases in our country. The key of controlling the increasing hypertension and conducting effective prevention and cure of the hypertension in the community is to establish an integrated health control mode, which adapts to the hypertension patients in critical conditions in the community so as to provide reference for the individual health control, the new form for the control of hypertension in the community, and the theoretical base for conducting the sustainable research of hypertension. In 2009, the system management method, which gives priority to the health education and behavior intervention, is used for the one-year integrated intervention and management of hypertension patients in critical conditions in the community of Anyang, so as to provide scientific bases for the prevention and cure of hypertension in the community.MethodThe center adopts the method of simple random draw among hypertension patients in critical conditions, whose health files have been set up, to choose 510 hypertension patients and 741 hypertension patients in critical conditions for the purpose of taking measures of one-year systematic integrated intervention and control, as well as questionnaire survey before and after the intervention. The main contents are as follows:Materials of current situations of perception, attitude and behaviors about situations of taking medicines, degree of smoking tobacco, capacity for drinking, the salt absorption, habits of athletic sports, as well as blood pressure control situations of hypertension patients.Result1. The awareness rate, regular blood pressure measurement rate, body weight control rate, diet control rate, sports motion rate, and timely taking medicine rate under the doctor's advice of diagnosis standard of hypertension patients after the intervention are far higher than those before the intervention, rising from 29.80%, 53.73%,34.12%,63.33%,61.18% and 66.67% to 70.59%,75.49%, 84.12%,85.88%, 75.10% and 80.59% respectively (P=0.0001).2. The awareness rate, regular blood pressure measurement rate, body weight control rate, diet control rate and sports motion rate of diagnosis standard of hypertension patients in critical conditions after the intervention are far higher than those before the intervention, rising from 9.99%,49.66%,43.32%,28.34% and 27.13% to 53.71%,73.82%,59.78%,38.06% and 43.59% respectively (P=0.0001).3. The blood pressure control rates of hypertension patients rise from 25.69% to 54.51%.ConclusionFocusing on the community, relying on the platform of community health service center, and commune integrated intervention of hypertension patients and hypertension patients in critical conditions, can remarkably increase the awareness rate of the patients on hypertension, the taking medicine rate of blood pressure reduction medicines and the blood pressure control rates, which has unique advantages and so is worthy of popularizing and applying in grassroots communities.
