Objective:There are two views about lymphatic metastasis of tumor. One is that cancer cells invade the lymphatic system surrounding the original tumor and form lymphatic metastasis. Another view is that in order to achieve tumor metastasis, cancer cells can produce some lymphokines and induce the formation of new lymphatic vessels. The former has been long dominant. In recent years, Lymphan-giogenesis with lymphatic markers gradually discovered begin to be taken seriously in tumor metastasis. The aim was to observe the D2-40 and LYVE-1 (lymphatic vessel endothelial hyaluronan receptor -1) labeled micro- -lymphatic vessel density (MLVD) in hepatitis B associated hepatocellular carcinoma, and theirs correlation and the determination of value. The other aims was to investigate the relationship between microlymphatic vessel density (MLVD) and hepatitis B-related liver cancer, and preliminary research the relationship between microlymphatic vessel density (MLVD) and Lymphatic metastasis.Methods:In this study, we selected 52 cases of hepatitis B virus associated hepatocellular carcinoma (HCC) as pathological specimens, and 10 cases of normal liver tissue. HCC specimens:lymph node metastasis were found in 19 cases, lymph node metastasis was not found in 33 cases; cancer tissue was well differentiated in 18 cases, moderately differentiated in 15 cases, poorly differentiated in 19 cases; in HCC staging,31 cases were in stageⅠ-Ⅱ,21cases inⅢ; hepatitis B viral load is more than 105IU/L in 23 cases, the others less than 105IU/L.HBVDNA<105IU/L 29 patients; rat anti-human D2-40 monoclonal antibody and rabbit anti-human LYVE-1 polyclonal antibody were separately detected,then counted their marked MLVD. In the last, statistical software SPSS 12.0 was applied for statistical analysis. Results:1. The microlymphatic vessel density (MLVD) in HCC marked by D2-40 was obviously higher than in normal liver tissue. In lymph node metastases group wasand in no lymph node metastases group14.49±2.17,10.31±3.28,Comparing between the two groupsP<0.05, the differences were statistically significant. MLVD in Liver well--differentiated and moderately groups was12.64±4.06, in poorly differentiated group 10.49±3.95, There were some difference among the two groups, P<0.05. MLVD in tumor-surrounding tissue of HCC was notably higher than those in tumor tissues. MLVD in HCC marked by D2-40 had nothing to do with patients age, gender, carcinoma diameter, clinical stages, HBVDNA and AFP. The differences were not statistically significant, P>0.05.2. The microlymphatic vessel density (MLVD) in HCC marked by LYVE-1 was obviously higher than in normal liver tissue too. MLVD in Liver well-differentiated and moderately groups was 8.86±2.78, in poorly differentiated group11.31±3.15. There were some differences among the two groups, P<0.05. In lymph node metastases group was 12.44±3.09, and in no lymph node metastases group was 8.21±1.58,Comparing between the two group (P<0.05), the differences were statistically significant. MLVD marked by LYVE-1 in HCC stageⅠ,Ⅱwas 12.97±4.17, and in stageⅢ9.51±3.42.There were some difference among the two groups, P<0.05. MLVD in Carcinoma tumor-surrounding tissue of HCC was higher than those in tumor tissues. MLVD in HCC marked by LYVE-1 had nothing to do with patients age, gender, carcinoma diameter, clinical stages, HBVDNA and AFP. The differences were not statistically significant, P>0.05.3 There was a positive correlation between MLVD in marked by D2-40 and marked by LYVE-1 52 liver cancer tissues. Both were of some value for predicting the lymphatic metastasis of the hepatitis B-related liver cancer, but the specificity of D2-40 was higher than LYVE-1 and the value of combined detection may be higher. Conclusion:1. D2-40 positive staining MLVD which is brownish yellow granules located mainly in the peripheral tissues of cancer, however cancer center lack of D2-40 or no. D2-40 labeled microlymphatic vessel density is closely related to the lymphatic metastasis and differentiation degree of hepatitis B-related liver cancer. It suggests that the formation of microlymphatic vessel density may be associated with hepatitis B liver cancer lymph node metastasis and disease progression, it is very specific and sensitive.2. LYVE-1 positive staining MLVD which is also brownish yellow located in the tumor interstitial lymphatics endothelial, or some tumor cells cytoplasm. LYVE-1 labeled microlymphatic vessel density may relate to the lymphatic metastasis and differentiation degree of hepatitis B-related liver cancer. It suggests that lymphatic vessel may be associated with hepatitis B liver cancer lymph node metastasis and disease progression. However LYVE-1 is not very specific markers for HCC.3. The LYVE-1 and D2-40 can more accurately reflect the situation of hepatocellular carcinoma microlymphatic vessel. It may have a certain diagnostic value for early liver cancer metastasis and spread.
