Objective:On the basis of inheriting and concluding the experience about the treatment about headache from Zhao Jinduo and combining with more than 20 years of clinical practice of nervous headache from other experts, observe the effect on tension-type headache treated with calming liver to endogenous wind following traditional Chinese medicine. Methods:Reference to the diagnostic criteria (1988) on recurrent tension-type headache from"the standard of International Headache Association, "and referring to national standards (1997) of " Clinic terminology of traditional Chinese medical diagnosis ", choose the patients belonging to the syndrome of hyperactivity of liver yang transformed into wind. We use the before and after self-control observation and pure herbal medicine to give prominence to the advantage of traditional Chinese medicine. The patients are given the herbal medicine following calming liver to endogenous wind, per 100ml, twice a day. Evaluation indicators include the number of headache attacks, days, the extent, duration, TCM-like (the main symptoms, minor symptoms) and so on. The treatment last 4 weeks. Each two week, above indicators are recorded. Markedly effective cases are followed up for 1 more month. Avoid other painkillers in the trial period.Result:1. The clinical effect of tension-type headache. The effective rate is 48.5% and the total efficiency is 78.8%. After four weeks'treatment, the intensity scoring of the nervous headache is obviously lower than before taking the medicine, including the headache attack frequency, headache days, headache duration, VAS score, the composite score and so on (P<0.05). And the classification on intensity of the nervous headache decrease significantly(P <0.05). The follow-up after 8 weeks, the intensity scoring and the classification on intensity of the nervous headache are significantly lower than beore treatment (P<0.05).2. The improvement of traditional Chinese medicine symptoms. The clinical cure rate is 6.1%, the effective rate is 12.1% and the total efficiency is 72.7%. After four weeks'treatment, compared with before taking the medicine, classification of Chinese medicine symptom is lower (P<0.05). And the scores of the distending pain of head, svere dizziness, limb numbness and other minor symptoms of traditional Chinese medicine decreased significantly(P<0.05). The follow-up after 8 weeks, the score of Chinese medicine symptoms (except limb numbness and blush) and the classification of Chinese medicine symptom are significantly lower than beore treatment (P<0.05).3. There were no significant adversary reactions in patients during the test process. Some patients were uncomfortable during the test process, eg. diarrhea, nausea and no desire to eat. But they were able to adhere to the medication. The conventional tests in patients including blood, urine, ECG, liver and kidney functions were normal. Conclusion:Calming liver to endogenous wind on nervous headache has achieved significant results. It can reduce the number of headache attacks, duration, and extent. It also can improve symptoms. Meanwhile, the high safety and less side effects of herbal medicine are sure. So, it has important clinical and social value using traditional Chinese medicine to treat nervous headache.
