Green Qin Treatment Of Acute Gouty Arthritis Experimental And Clinical Research

My thesis includes three parts:literature summarization, experimentalresearch and clinical observation.The part of literature summarization reviews the progress of research on treating acute gouty arthritis including the aspects of Western Medicine and Traditional Chinese Medicine (TCM). This part reflects the status quo of researches on epidemiology, pathogenesis, pathology, diagnosis, and treatment. It also summarizes the knowledge of scholars of TCM on the name and cause of the disease, pathogenesis, treatment of TCM.. We find westen medicine's weak points and TCM's advantage. Professor Meng Fengxian think insufficiency of the liver and the kidney, damp-heat, blood stasis is the cause of the disease, and composed QingQinYe. satisfactory effects have been achieved.The experimental study included observation of QQY treating the acute gouty arthritis rats.70 Wister animals were divided into normal group, model group, positive medicine group, Qing Qin Ye high dosage group, Qing Qin Ye middle dosage group and Qing Qin Ye low dosage group. Adenine and ethambutol manufactured model by gavage. Colchicine 0.09 mg/kg·d were administered to the rats of positive medicine group by gavage. Rats of Qing Qin Ye high dosage group were administered Qing Qin Ye 5 g/kg·d, middle dosage group were 2.5 g/kg·d and low dosage group were 1.25 g/kg·d by gavage. Determine the level of serum uric acid by phosphotungstic acid method. Determine the level of XOD and ADA by enzymic colorimetric method. The results indicated that Qing Qin Ye could decrease the level of serum uric acid because of decrease ADA and XOD activity in rats acute gouty arthritis model.The clinical study included systematical clinical observation of QQY treating acute gouty arthritis. According to diagnostic criteria, the cases had been sifted. 108cases had been collected, who were divided into treatment group 50 patients and control group 58 patients. Drug treatment group used the QQY. The control group used oral drug, Simiao pill. Two weeks of treatment. Before treatment and after treatment were observed in patients with symptoms and signs of Chinese medicine points, joint function classification, white blood cell countsl, creatinine, urine protein, adverse reaction. The results indicated that the QQY can significantly improve symptoms, signs and joint function of acute gouty arthritis patients (P<0.05). QQY can decrease the level of serum uric acid, and the white blood cell counts. Treatment groups compared before and after treatment, the differences were statistically significant (P<0.05), the treatment group compared with the control group,with statistical analysis P>0.05,no significant difference, suggesting that QQY and Simiao pill have similar improving effects on serum uric acid, and the white blood cell counts. Treatment groups compared before and after treatment in decrease creatinine, the differences were statistically significant (P<0.05), so QQY can decrease creatinine. While Simiao Pill groups compared before and after treatment, the differences were not significant (P>0.05), so QQY can decrease creatinine. Both the two groups were not significant differences compared with before in decrease urine protein(P>0.05). The total effect between the two groups after treatment showed no statistically significant difference (P>0.05), so QQY and Simiao Pill were similer in treating acute gouty arthritis. No obvious adverse reaction and side effect were observed in the two groups.Conclusion:Qing Qin Ye could decrease the level of serum uric acid because of decrease ADA and XOD activity in rats acute gouty arthritis model. And QingQin Ye can significantly improve symptoms,signs, significantly reduced in patients with serum uric acid,creatinine, white blood cell.
