Objectives:To evaluate the changes of vascular morphology,homodynamic and endothelial function in patients with early-stage type 2 diabetic mellitus(T2DM) by highresolution ultrasonography.Methods:1.Measuring the intima-medial thickness(IMT) of the carotid artery,femoral artery,iliac artery and the changes of the diameter of brachial artery under reactive hyperemia and after sublingual nitroglycerin in 22 elderly early-stage T2DM patients, 142 non-elderly early-stage patients with T2DM and 20 elderly and 20 non-elderly healthy persons.2.Measuring the changes of vascular morphology,homodynamic and endothelial function of the dorsalis pedis arteries in 25 elderly early-stage T2DM patients,35 non-elderly early-stage T2DM patients,35 elderly and 30 non-elderly healthy persons.Results:1.The increased percentage of the brachial artery diameter under reactive hyperemia was significant lower in elderly early-stage T2DM patients(10.03±6.39)% than in the elderly healthy controls(15.87±4.79)%,and was significant lower in non-elderly early-stage patients(13.38±5.94)%than that in the non-elderly healthy ones(16.91±5.97)%(P<0.05).There was no significant difference in the changes between the two groups in response to nitroglycerin(P>0.05).The IMT in elderly early-stage T2DM patients incrassated significantly(P<0.05).2.①The increased percentage of the dorsalis pedis arteries diameter under reactive hyperemia was significant lower in the elderly T2DM patients(10.52±2.79)%than in the elderly healthy controls(15.43±4.69)%,and there was significant difference between the non-elderly T2DM patients(12.89±4.68)%and the non-elderly healthy persons(17.97±4.61)%(P<0.01),and in the elderly T2DM patients was also lower than in the non-elderly T2DM ones(P<0.05).The increased percentage of the dorsalis pedis arteries diameter was significant lower in the elderly T2DM patients(12.40±3.23)%than in the elderly healthy controls(16.11±5.74)%in response to nitroglycerin(P<0.01).②Compared between the patients with early-stage T2DM and the control groups,the intima-medial thickness(IMT) incrassated significantly(P<0.01),peak reverse velocity(PRV) and end-diastolic velocity(EDV) reduced significantly(P<0.01), pulsatility index(PI) decreased(P<0.05).The peak systolic velocity(PSV) had no significant difference among all the groups(P>0.05).③The PI correlated positively with the increased percentage of the dorsalis pedis arteries diameter under reactive hyperemia(r=0.79,P<0.01) and in response to nitroglycerin(r=0.71,P<0.01)in patients with T2DM.Conclusions:The injuries of vascular morphology,homodynamic and endothelial function can be detected in early-stage of T2DM by high-resolution ultrasonography which has clinical significance and applicable value.
