Objective:To evaluate retrospectively the impact of Tension-free Vaginal Tape(TVT) and TVT-Transobutor system(TVT-O) on sexual function using validated questionnaire after placement of a distal urethral polypropylene slingMeterial and methods:This study included 110 patients(range of age 42-81,mean age 54±10.3) who underwent TVT from September 2002 to December 2004 and 72 patients(range of age 36-72,mean age 53±8.7岁) who underwent TVT-O from January 2005 to March 2008.The patients were divided in six groups based on the operation style and the number of sexual lives.The patients were evaluated with Female Sexual Function Index(FSFI) questionnaire which assesses six domains of sexual function:desire,arousal,lubrication,orgasm,satisfaction,and pain.The t-test were performed to determine the difference of FSFI scores.Results:1.The effective rate of TVT and TVT-O are 93.6%and 98.6%.TVT incurs higher prevalence of complications compared with TVT-O and the most common complications of both surgeries are frequency,urgency and dysuria.2.In 167 patients,98 patients(89.1%) who performed TVT surgery and 69 patients (95.8%) who performed TVT-O surgery agreed to participate in this prospective study.89 patients(53.3%) with SUI reported that they do not have sexual activity preoperativly and postoperatively.No statistically significant difference was found both in tltal and 6 domain in 28 patients(16.8%) who have sexual activity more than once a week and in 50 patients(29.9%) who have sexual activity occasionally once a week.2 patient who were afraid of urine leakage preoperatively reported improvement during sexual activity after operation,the FSFI scores rise from 13,15 to 28,23.3.No statistically significant difference was found both in tltal and 6 domains of FSFI in 28 patients(16.8%) who have sexual activity more than once a week and in 50 patients(29.9%) who have sexual activity less than once a week.4.No statistically significant difference was found both in tltal and 6 domains of FSFI in the patients without hysterectomy and hysteroptosis and kysthoptosis5.No statistically significant difference was found both in tltal and 6 domains of FSFI in the patients who underwent TVT or TVT-O no mater how many times they have sexual activities.Conclusion:1.TVT and TVT-O are both safe and efficient for teating females with SUI.2.TVT and TVT-O surgery neither improve sexual function nor impair sexual function.However,the quality of sesual activity may be improved for the patients who were afraid of urinary leakage before TVT and TVT-O surgery.
