This paper includes three parts:A Literature Review,Theory of Chinese medicine and experimental research.1 Literature ReviewFirst of all,we highlight these new developments in epidemiology,pathogenesis, pathology change,clinical diagnosis and treatment of the reflux esophagitis(RE),in which discuss the best therapy project and research directions of the RE in the future. Second,the gastrointestinal hormones contact with RE.Summarise the latest research progress between three kinds of gastrointestinal hormones(gastrin,motilin and vasoactive intestinal peptide) and the reflux esophagitis in detail.Finally,the traditional Chinese medicine(TCM) has studied in RE recently.The experiences of TCM in RE were summarized,including of pathogeny and mechanism,clinical thought of the differentiated and therapy.2.Theory of Chinese medicine2.1 Explaination and application of the ancient medical scientists on Zhizi Gancao Chi Tang(ZGC)Investigate the origin of ZGC,interpretate ZGC and use ZGC in ancient times. At the same time,discusses the ancient interpretation of Zhizi Chi Tang(ZC).2.2.Explaination and application of the ancient medical scientists on Dahuang Huanglian Xie Xin Tang(DHXX)Investigate the origin of DHXX,interpret DHXX and use DHXX in ancient times.Notes on the basis of medicine,I try to get more comprehensive understanding of ZGC and DHXX and use them in flexibility.3 Experimental research3.1 ObjectiveSimulation of reflux esophagitis model in rats to observe the effects of Zhizi Gancao Chi Tang(ZGC),Dahuang Huanglian Xie Xin Tang(DHXX) and ZGC+DHXX in rat models with the reflux esophagitis(RE) and explore the mechanism,in order to provide scientific basis for clinical treatment.3.2 MethodsA total of 72 male SD rats were randomly divided into 6 groups: sham-operated control group,model group,the western medicine group,ZGC group,DHXX group and ZGC+DHXX group.The rats in control group underwent sham operation.The other rats underwent esophagoduodenostomy to establish the model of RE.After 2 weeks,the rats in sham-operated control group and model group were given equivalent volume of normal saline daily,the others were given respective drugs:Mosapride 1.75mg/kg,ZGC 5.83g/kg,DHXX 3.62g/kg and ZGC+DHXX 4.78g/kg.All the rats were weighed once a week while they were fasting.All rats were sacrificed at 5 weeks after operation.The whole esophagus was weighed to calculate the esophagus coefficient.The changes of RE mucosa (both macroscopic and microscopic) were observed and identified.The contents of activity of SOD and MDA content and NO concentration in esophagus tissue homogenate were measured.We also measured GAS,MILand VIP levels,and SOD activity,MDA and NO in serum.3.3 ResultsThan the model group,ZGC group and ZGC+DHXX rats increased body weight significantly.ZGC group and DHXX group and ZGC+DHXX group all can decrease the esophagus coefficient and reduce the degree of the esophageal mucosa injury significantly.The effect of ZGC+DHXX group was the most obvious.ZGC group,DHXX group and ZGC+DHXX group can significantly reduce MDA content esophagus,NO concentration and raise SOD vitality in serum and in esophageal tissues,in order to protect esophageal mucosal.The western medicine group,ZGC group,DHXX group and ZGC+DHXX group are able to raise serum levels of GAS and MIL.Reduced serum VIP,in which ZGC+DHXX group is superior to the positive control group Mosapride,so as to enhance the lower esophageal sphincter pressure.3.4 ConclusionsIn this experiment,ZGC group,DHXX group and ZGC+DHXX group can significantly reduce the extent of esophageal mucosal injury in reflux esophagitis model rats,in which the most obvious effect is DHXX group.Mechanism may be through:(1) Three groups all significantly increased the level of SOD body,lower the MDA content in esophageal tissues and in the body,so as to reduce the oxygen free radical damage to the esophageal mucosa and protect the esophageal mucosal.(2) Regulation by reducing the enteric nervous system non-adrenergic non-cholinergic(NANC) neurotransmitter levels of NO and VIP to reduce NO and VIP on lower esophageal sphincter relaxation and thus the relative enhancement of lower esophageal sphincter pressure to put an end to the stomach anti-intestinal contents into the esophagus,protect esophageal mucosal.(3) The impact of gastrointestinal hormones GAS and MIL.By increasing the level of GAS and MIL to strengthen lower esophageal sphincter pressure to stop the stomach contents flow into esophagus and to protect esophageal mucosal. Mechanism of traditional Chinese medicine is often multi-site,multi-link, multi-target,so study of Chinese medicine treatment of reflux esophagitis have broad prospects.
