Hypertension Blood Stasis Syndrome And Correlation Of Ambulatory Blood Pressure

Clinical investigationObjectivesTo investigate the characters of the symptoms of the blood stasis syndrome in hypertensive patients,mainly in different age grades,different course grades, different BP levels,different cardiovascular risk stratification,other types of syndrome which it combine with.And then illuminate its disposition rule.To investigate the characters of ambulatory blood pressure of the blood stasis syndrome in hypertensive patients,Including average blood pressure levels,blood pressure variability,blood pressure load,Circadian rhythm of blood pressure and so on.MethodsHypertensive patients over the age of 18 years and below 75 years,attending outpatient clinics and inpatients of guang'an men hospital of general internal department were interviewed.The diseases types of the patients were among hyperactivity of liver-yang syndrome,yin deficiency of liver and kidney syndrome, liver-wind and phlegm syndrome and deficiency of qi and yin syndrome.They were also classified with the blood stasis syndrome or non-blood stasis one.We make clinical investigation through questionnaire which was made by us and medical examination.Ambulatory blood pressure monitoring should be done for inpatients.Trained doctors completed questionnaires regarding demographics, treatment of history,symptoms,physical sign and so on.Data was managed and statistically analyzed by software SPSS 13.0.Two-sample t test or the One-Way ANOVA tested normal distribution data.Non-normal distribution Data were tested by NPAR test,such as Mann-Whitney test,Kruskal—Wallis test,Chi-square analyses.ResultsA total of 286 hypertensive patients were enrolled;189(66.1%) had blood stasis syndrome.The result showed that the incidence and degree of the blood stasis syndrome in hypertensive patients were related to age,course,cardiovascular risk stratification and other types of syndrome they belong to.They aggravates along with the increasing of age,course,or cardiovascular risk stratification,they are higher in syndrome of liver-wind and phlegm syndrome or deficiency of qi and yin syndrome than hyperactivity of liver-yang syndrome,yin deficiency of liver and kidney syndrome.Systolic blood pressure,systolic blood pressure variability,systolic blood pressure load,incidence of Circadian rhythm of blood pressure disorder of the blood stasis syndrome in hypertensive patients were higher than non-blood stasis ones.ConclusionsComparing symptoms and physical sign between the blood stasis syndrome and non-blood stasis one,we induce the characters of the symptoms of the blood stasis syndrome in hypertensive patients.It includes headache,cervical part stiffness,chest distress,chest pain,palpitation,extremity Anaesth,hemiparalysis,dark red of oral lip,orbit,palm of hand and body of the tongue,unsmooth pulse.Doing further analysis,we think there are three aspacts of the characters of the symptoms.Firstly, the syndrome can be divided into principal one and minor one.Principal syndrome includes headache,cervical part stiffness,extremity Anaesth,dark red of oral lip and body of the tongue,unsmooth pulse;while minor syndrome includes chest distress, chest pain,palpitation,hemiparalysis,dark red of orbit and palm of hand.Secondly, there is rule of the syndrome's variance.For people who are young,whose course is short,or with low cardiovascular risk stratification,the syndrome only includes principal one.For people who are old,whose course is long,or with high cardiovascular risk stratification,the syndrome includes both principal one and minor one.Thirdly,the syndrome is different when the blood stasis syndrome is combined with other syndrome of hypertension.When it is hyperactivity of liver-yang syndrome or yin deficiency of liver and kidney syndrome,the syndrome is light,but when it is liver-wind and phlegm syndrome or deficiency of qi and yin syndrome,the syndrome is serious.It is considered that not only average blood pressure levels,blood pressure variability,but also the blood pressure load,Circadian rhythm of blood pressure are related to the target organ damage in hypertensive patients.We should pay enough attention to the control of those indexes in the therapy just in order to avoid or ease the target organ damage in hypertensive patients.
