Diabetic Patients With Depression In Traditional Chinese Medicine Syndrome Clinical Study Of The Characteristics And Influencing Factors

Diabetes is a chronic and life-time disease .With the increase of age,the incidence of it work up.Beacause the diabetes can not be cured and needs lifetime drug treatment, and the delivery pressure of people become larger nowadays,this makes the correlation between diabetes and depression become more and more close.Besides the complication of it,more patients psychologic status changes, depressive disorder happens.With the lead of chinese medical science theory, to study and research diabetes merge depression,we hope probe the Chinese Medical Symptomatology on diabetes merging depression, to analyze the influencing factor.Thus to supply directry intervention study and achive the purpose of improving life quality.This thesis is made up of two parts:literature review and clinical research. In the first review named The research advancement of chinese medicine on diabetic merging depression.It introduces the the research advancement of chinese medicine on diabetic merging depression in resent years.It contains interpretation of the cause , differentiation of symptoms and signs for classification of syndrome ,relation between chinese medical symptomatology and the degree of depression and pretreatment on diabetic merging depression.The second review the research advancement of modern medicine on diabetic merging depression and influencing factor in resent years. It contains interpretation of the cause , clinical feature , disease rate, dangerous factors, relation between diabetes and depression,treatment, psychologic status and influential factor on diabetes merging depression.The clinical research part investigate 200 diabetic patients, all cases originate from July 2006 to December 2007 in Beijing Chinese medicine University East Hospital endocrine in-patient. According to the Hamilton despondent meter (HAMD) and the diabetic patient clinical observing table, we gathering case and the questionnaire survey. The general state of health including the name, the sex, the age height, the body weight, the family history, the financial circumstance, the education level, the child number and so on. Data correlating diabetes include diabetes course, (FPG), meal latter 2 hours blood sugars (2hPG), saccharification hemoglobin (HbA1C), blood fats, body weight index (BMI), whether using insulin treatment and so on. In addition,it also includes diabetes nephrosis,diabetes retina pathological change, diabetes foot, blood vessel pathological change, diabetes peripheral nerve pathological change and so on. All data use the application to carry on statistics and analysis with the SAS medical statistical software. The counting material uses the chi-square test, the measurement data uses the T-test.The research result shows: 1. In the middle of this 200 example diabetic, type of deficiency of both QI and YIN occupy the first place, altogether 51 examples, the incidence rate is 25.5%; After that is in turn the stagnation of QI due to depression of the liver > extreme heat with yin asthenia > the vein stasis . In the middle of the diabetes patient merging depression, the stagnation of QI due to depression of the liver occupies 38 examples, the non-depression group is 11 examples, the two exist remarkable difference. 2. the moderate-depressive group has stagnation of QI due to depression of the liver is higher than the mild-depressive group, the severe-group is higher than the mild-depressive group and the moderate-depressive group obviously. 3. Female's depressive disorder incidence rate is higher than the male obviously, the old age patient (the >60 year old) depressive disorder incidence rate is higher than ( <60 year old) group, the university group depressive disorder incidence rate is higher than below -group. 4. Compared with diabetes non-depression group, in diabetic patient merging with depression the female is more than the male obviously, the years of schooling is higher, the saccharification hemoglobin is higher,using the insulin treatment, the chronic complication is more, both difference has the significance. 5. With the diabetic depressive symptom aggravates, the saccharification hemoglobin elevates, the diabetic chronic complication increase, the course will be long.Betweenâ… Group ,â…¡Group andâ…¢Group,â…¡Group andâ…¢group ,the difference has the significance.
