Objective: To review the relevant articles about treatment of Hallux Abducto Valgus(HAV) in the past in order to know the progress of the remedy for the disease.Method: To classify and analysis the different HAV treatments by case reports, randomized trails and overviews from CNKI and MEDLINE. The treatments for HAV can be divided into two parts: conservative and surgical treatment which was highlighted in this article. The surgical treatment mentioned were as follow: distal soft-tissue reconstruction, distal metatarsal osteotomy, diaphyseal metatarsal osteotomy, basal metatarsal osteotomy with distal soft-tissue reconstruction, medial cuneiforme osteotomy, proximal phalangeal osteotomy(Akin procedure),the first metatarsophalangeal joint procedures, arthrodesis of the metatarsocuneiform joint and minimal incision technique. Those procedures were described and compared with respects of indications, surgical steps, nursing, satisfaction levels and complications. The minimal incision technique was noted here in order to introduce its theory of integrated Western and Chinese Medicine and its specific procedures. On the other hand, the systematic review of interventions for treating HAV and bunions was picked up from Cochrane Database to learn the quality of randomized trails of HAV, and its conclusions were also compared to others'.Result: There are plenty of treatments of HAV reported, which resemble in indications and procedures, but are different from time and ways of fixation, exercise, complications and post-operative satisfaction levels. The main results of the systematic review were as follow: A. Evidence from randomized studies for the effectiveness of treatment for HAV is limited by a paucity of good quality, large size trails. B. One good study comparing surgical correction of HAV with conservative treatment or no treatment found in favour of the surgical treatment. C. There was no evidence that any one type of surgical procedure was superior to another across a range of outcomes.Conclusion: A. Every single treatment can obtain a good result as long as its advantages and disadvantages are well recognized and conducted. B. The minimal incision technique invented by Pro.Wen Jianmin has improved the method of treating HAV. The Traditional Chinese Medicine theories, including balance of Yin and Yang, soft-tissue restricting bones and concern on both soft-tissue and bones, are applied in this method and made this treatment more effective. |
