| Objective:To observe the clinical curative effect of treating peptic ulcer(PU)'s recrudescence with yu-yang-fang and explore the mechanism of treating peptic ulcer's recrudescence with yu-yang-fang from theoretical and clinical aspects.Method:1.68 cases were patients in March 2006 to May 2007 between Hubei traditional Chinese medical college's affiliated hospital in the digestive internal medicine out-patient treatment of cases,all cases have been confirmed in our hospital for peptic ulcers,and after the treatment system,as gastroscopy The patients have been healed. The gastroscopy confirmed for ulcer recurrence,and Hp test was negative,TCM spleen deficiency of the Liver Depression.According to the order in attendance were divided into 38 cases of the treatment group and control group of 30 cases.In the two groups in age,gender,duration,severity and treatment before symptoms total score of Chinese medicine,ulcer treatment relapse site and staging areas gastroscope there was no significant difference,comparable(P＞0.05).2.Chinese medicine for the treatment group Yu-yang-fang side,the side to the experience of instructors(from Astragalus 15 g,dangshen 15 g,Bupleurum 12 g, Baizhu 15 g,Zhishi 15 g,baishao 15 g,Banxia 10 g,Dried Ginger 10 g,Huanglian 6 g,Salvia 15 g,Bletilla striata 15 g,composed of licorice 6 g),the drug Shui Jianfu, one day,sooner or later the service.At the same time give famotidine 20 mg,every night before bed medication.Control group:20 mg of famotidine,taking every night before bed.The two groups are taking two,one on a course of treatment,continuous observation of the two courses were conducted safety of(blood,urine,fecal three regular checks liver and kidney function;electrocardiogram;possible adverse reactions)and the effect of Observation(signs and symptoms of observation, gastroscopy).3.Were compared between the two groups before and after treatment of Chinese medicine syndrome total score,after the comprehensive effect of the treatment, treatment before and after the clinical symptoms score,the two groups after treatment ulcer relapse rate after treatment of ulcer recurrence of endoscopy.4.Statistical analysis:data applications SPSS 13.0 statistical software for analysis.Rating information with the Ridit test,x~2 classified information with the inspection,measurement data used t-test. Results:1.The treatment group of 38 cases,clinical cure five cases,12 cases markedly effective,effective in 12 cases,three cases invalid,and the total efficiency of 92.11 percent.The control group of 30 cases,one case of clinical cure,seven cases markedly effective,effective in 12 cases,10 cases of invalid,and the total efficiency of 66.67 percent.Clinical cure rate of the treatment group,significant efficiency,the total efficiency of the control group(P＜0.05).2.After treatment Syndrome total score two more very significant difference(P＜0.01);group of Chinese medicine treatment after symptoms of the total score improved significantly better than the control group(P＜0.05).3.Treatment and control groups were treated before and after the clinical symptoms integral statistical analysis(t test),there was significant difference(P＜0.05),prompt treatment and control groups of clinical symptoms significantly improve the effectiveness;two Group after treatment results statistical analysis(t test), have different levels of statistical difference,can significantly improve the treatment group Weiwan pain,Xiongxiezhangman,Aiqi,drinking less Nacha,burnout fatigue, abdominal distention,and other symptoms,The effect is better than the control group, but they Tang on the improvement and effect of a control group.4.Treatment and control groups were treated before and after the recurrence rate of the statistical analysis(t test),there was significant difference(P＜0.05),prompted anti-ulcer treatment group recurrence of significantly better than the control group treatment.5.After two cases of recurrence of ulcers compared to endoscopy statistical analysis,a very significant difference(P＜0.01);treatment group not only the low rate of relapse,and relapse after endoscopy of the clear light on the extent of the control group,with A very significant difference(P＜0.01).Conclusion:The research suggested that combining traditional Chinese and western medicine anti-peptic ulcer recurrence of better than Western medicine.Yuyang Liver depression can significantly improve the spleen deficiency of peptic ulcers in patients with various clinical syndromes,relieve pain,improve the quality of life;Yuyang,the mechanism may be through raising peptic ulcers in patients with immune cells and humoral immunity,and enhanced Gastric mucosal barrier protection and regeneration of epithelial cells;To the eradication of Hp,increase gastric prostaglandin synthesis, enhance gastric barrier to protect against gastric acid and pepsin,and other attacks of the invasion;increase the cycle of gastric mucosa,perfusion,and promote their energy metabolism,and guarantee epithelium Ulcer at the bottom edge of glands and the rapid proliferation and maintain the mucous membrane of the microcirculation of acid-base balance,so as to improve ulcer healing quality,reduce ulcer recurrence. Therefore,we have to play the advantages of Chinese medicine,better for Integrated Treatment,and optimize the anti-peptic ulcer recurrence of treatment programmes,to promote Chinese medicine in the academic level of development and to improve our people's health and hygiene standards. |
