ObjectiveDiabetes mellitus (DM) is a commonly seen systemic disease marked by genetic tendency and disturbance in both glucose metabolism and endocrine function, and also a chronic, life-long and intractable disorder. Its complications can damage various visceral organs of the body, thus seriously affecting the quality of life of the patients, and finally leads to disability or even endanger the lives of the patients. According to WHO statistics, now there are global diabetic patients of more than 194 million, and DM has become the world's fifth cause of death. In our country the prevalence of DM is about 25‰of Chinese population, increases at an annual speed of 0.1 percent, and may have an annual 754,000 new cases of DM in the people with age between 25 and 74 if Chinese population is supposed as 1300 million while starting the 21st century.It is currently thought that Western medicine is more effective and quicker in controlling patient's blood glucose, while Chinese herbal medicine shows a certain advantages in improving patient's symptoms, raising the quality of life, and preventing-treating the complications, and moreover, it has been found that the treatment of TCM has multifaceted, multi-level, multi-link adjusting actions on the DM patients. However, based on the existing reports, there is a lack of unified classification of syndrome patterns in the clinical observations on treating DM with Chinese herbal medicines by the various TCM organizations, and the contrasts of therapeutic effects between Chinese herbal decoction or patent and Western medicine were made in the most reports, therefore, the great differences of therapeutic results appeared in the statistical data of the reports, and further credibility of the studying conclusions in these reports was doubted too. To try to explore a new mode and method for applying Chinese herbal medicine, this research first uses Chinese herbal pathogenesis preparations put forward by professor Cheng Zhao Zhi in clinical observation of the patients of type II of DM so as to greatly enhance and widen the application of TCM in treatment of DM and the other diseases presupposing the preserving the TCM advantage of Bian Zheng Shi Zhi, and really promote TCM going into the whole worldMethodsAll 32 cases of type II DM here are the outpatients from department of endocrine disorders in Hubei Provincial TCM Hospital. According to the diagnostic standard of DM in New Guiding Principles for TCM Clinical Study worked out by Ministry of Health of the People's Republic of China in 1993, and combined with the patients ' sick history, symptoms and signs, they were divided into different syndrome patterns, namely, four basic pathogenesis, Qi deficiency, Yin deficiency, blood stasis and phlegm retention as well as their concomitant patterns. Then each patient was treated with the relevant preparation for continuous four weeks as one course of treatment. The preparations given were modified timely according to the therapeutic effects and changes of clinical manifestations a week after taking the preparations. The therapeutic evaluation was made unitedly after finishing the treatment course according to the diagnosing-treating standard in DM Clinical Standards of Integrated Chinese and Western Medicine (Draft) worked out by DM professional committee, Chinese Association of Integrative Chinese and Western Medicine in 2005 All of symptoms, signs and levels of both fasting and postprandial blood glucose before taking the medicines and after every one week were observed and recorded on time. Afterward, the final statistical processing and analysis were made, and further the advantages and shortcomings of the mode and method of new TCM treatment were discussed.Results1,The appraisal of the four Chinese herbal preparations (granules) with thin layerchromatography indicated the quality of their raw materials have been assured.2,Efects: After a course of treatment, The total effective have reached the rate of 84.4percent.3,Blood glucose:After treatment fasting glucose and 2 h postprandial glucose wassignificantly lower than before treatment,the difference was significant (P <0.05)4,Symptom : After treatment the symptoms of integral value have been lowed.Compared before and after treatment,the difference was significant (P <0.05)Conclusion1, The quality of the Chinese herbal pathogenesis preparations (granules) has been assured, and they are quite convenient to use.2, The pathogenesis preparations have been approved to be definitely effective in both improving the clinical symptoms of DM and lowering blood glucose.3, Generally speaking, the four kinds of pathogenesis preparations are not enough in both strength and speed of reducing blood glucose, so should be risen in the future.4, As a new type of TCM diagnosing-treating mode and applying Chinese herbal method for DM and the other diseases, the TCM pathogenesis preparation has shown an organic combination between the TCM feature of Bian Zheng Lun Zhi (actually, treatment aiming at pathogenesis ascertained) and conveniently used new modern Chinese herbal preparation, and consequently is worth of being researched and perfected in the future.
