Objective: To explore the mechanism of action and clinical effects of traditionalChinese medicine preparations in treatment of diabetes mellitus, the effects on relatedindicators of combination medication with Chinese medicine compound formulaJianyutangkang tablet and metformin in treatment of type2diabetes mellitus wasanalyzed, and the clinical practice of traditional Chinese medicine injection which issalvianolate injection used in hospitalized diabetics was observed.Methods:1.By conducting a randomized blinded controlled trial,112type2diabeticpatients treated with metformin were randomly divided into treatment group andcontrol group,59cases in the treatment group added taking Jianyutangkang tablet,while53cases in the control group taking placebo. The treatment lasted for26weeks.Glycemic level, blood lipid, hepatorenal function, hematologic parameters,inflammatory factors, oxidative stress parameters and vascular endothelial functionwere measured before and9,18and26weeks after the treatment, and compared in thesame group and between two groups.2.By the method of real world study, the clinicalinformation of the diabetics treated with salvianolate injection in our hospital fromAugust2012to January2013was tracked and analyzed, which included general data,diagnosis, medical advice, laboratory and assistant examinations, adverse reaction, etc.Results:1.After26weeks combination treatment with Jianyutangkang tablet andmetformin, the level of fasting plasma glucose(FPG), glycosylated hemoglobin Alc(HbAlc), total cholesterol(Tch), triglyceride(TG), low density lipoprotein cholesterin(LDL-C), lipid ratios(Tch/HDL-C, LDL-C/HDL-C), tumor necrosis factor-alpha(TNF-α), interleukin-6(IL-6), maleic dialdehyde(MDA), total superoxide dismutase (T-SOD), and vascular endothelial growth factor(VEGF) of the treatment groupimproved significantly when compared with control group and themselves beforetreatment(P<0.05).2.A total of345diabetic in-patients were enrolled, most of whomwere overweight middle-aged and elderly people mainly in the department ofcardiology and vascular surgery. The main coexistent diseases were coronary heartdisease and hypertension. The salvianolate injection treatment was basically accordedwith the specification. The safety of the drug was satisfied as well. The combinedtreatment were mostly with cardiovascular and hematological drugs. In addition, theclinical indicators related to diabetes improved after the treatment.Conclusion:1.The combination treatment with Jianyutangkang tablet and metforminhad better therapeutic effect than the metformin monotherapy on type2diabetesmellitus. It could not only decrease blood sugar efficiently, but also ameliorate the lipidmetabolism disorder, suppress inflammatory state and oxidative stress and improvevascular endothelial function of type2diabetic patients.The results showed that thecombined treatment of traditional chinese medicine and western medicine on diabeteshad special advantages. Traditional Chinese medicine preparations were considered tohave the function of improving physical condition comprehensively, regulatingmetabolism balance and increasing the hypoglycemic activity of western medicinesynergistically. So that the combined treatment could be more effective to prevent andtreat diabetes and its complications.2.The real world study is effective for clinicalresearch of traditional Chinese medicine. The administration of salvianolate injectionwas rational in our hospital. And the improving trends of related clinical indicatorssuggested that it might be beneficial to diabetes and its complications. This study hadgathered experience for the investigations of clinical rational drug use and providedsome references for combined treatment of Chinese and western medicine of diabetesand its complications.
