| 1. BackgroudThe pneumonia induced by antibiotic-resistant bacteria is is a serious clinical problem. The presence of antibiotic-resistant pathogens limits the use of antibiotic agents in treatment of diseases.Antibiotic-resistance now occurs in an increasing number of pathogenic, opportunistic, and commensal bacteria.If the current trend continues, many of the commonly used antibiotics will soon lose their effectiveness.The older is old with many diseases and resistance and immunity are bad which make the cure of the pneumonia induced by antibiotic-resistant bacteria more dfficultThe pneumonia induced by antibiotic-resistant bacteria of the Elderly belongs to wind - warm fei - heat disease in traditional Chinese medicine. We have done a lot of work in wind - warm fei - heat disease ,but we don't research a lot to a specila type wind - warm fei - heat disease,that is The pneumonia induced by antibiotic-resistant bacteria of the elderly.Therefore ,it's very important to research the characteristic of intestine disease and Symptom Complex and the rule of disease development .2. ObjectiveThe objective of this research is to discuss the characteristic of intestine disease and Symptom Complex and the rule of disease development to guide clinical therapy by analysing 65 cases of the pneumonia induced by antibiotic-resistant bacteria and 65 cases of Pneumonia showing negative in Sputum Culture in the Elderly.3. MethodReview 65 medical records of the pneumonia induced by antibiotic-resistant bacteria of the Elderly and 65 medical records of negative Sputum Culture of Pneumonia of the elderly, fill in the observation report, put down the main symptoms, secondary symptoms, physical sign and laboratory result in Differentiation of Syndromes of Wei, Qi, Ying and Xue Systems.4. Result4.1 entirety condition:at the death rate ,the incidence of coma, time in hospital ,the incidence of hospital infection and the number of basic illness, especially the tumor and cerebrovascular disease,antibiotic-resistant bacilli Pneumonia is more than negative Sputum Culture of Pneumonia of the Elderly .Comparing the 2 groups , all of the above results were significant in statistics.4.2 Characteristic of symptom-complex4.2.1 The frequency occurrence of symptoms of Weifen which was more than 30% included: cough(72.1%), white sputum(64.0%), fever(44.0%), pant(32.0%).The picture of tongue and the pulse tracing were not collected completely so it were not discussed here.4.2.2 The frequency occurrence of symptoms of qifen according main symptoms, secondary symptoms, picture of tongue, pulse tracing and examination of blood was as follow:①Mainsymptom: more cough and more moderate cough, more sputum; more white sputum, yellow sputum and phlegm; more fever.There is statistical significance between positive group and negative group in the high of fever and the former is higher. There is no statistical significance between positive group and negative group in other sputum culture. ②The frequency of occurence of secondary symptoms was: poor food intaking, tired short breath, dry stool, fewer urination, aconuresis, mouth dry and thirsty. Athe frequency of occurrence in symptom of aconuresis, tired and short breath, positive group was higher than negative group in sputum culture.③In terms of body of the tongue dark red dominated. In coated tongue more were few mosci, yellowish greasy tongue, white and greasy fur, the frequency of occurrence of few mosci in positive group was higher than negagtive group.④In pulse tracing, more were wiry and slippery pulse, slippery and fast pulse.There is no statistical significance between positive group and negative group⑤In blood examination, more were normal or high in neutronphils. The frequency of occurrence of normality or hight in neutronphils white blood cell in positive group is higher than the negative. The frequency of occurrence of hight in WBC is opposite.4.3.3 yingfen syndrome①The frequency occurrence of yingfen syndrome in positive group is higher than the negative.②Mind:the frequency occurrence of coma came afterward is higher which have statistical significance compared with negative group.③The frequencies of occurrence of other symptoms of ying were: Night fever,wheezy phlegm in the throat,dry stool,cough,fewer urination,aconuresis,poor food intaking,disorder of four limbs,difficult to speak,pant.It had no statistical significance compared with negative group.4.3.4 faint and depletion syndromeThe frequency occurrence of faint and depletion syndrome in positive group is higher than the negative. The frequency occurrence of death has no statistical significance between two groups.4.3.5 Deficient of vital QI leading to lingering of pathogen①degree of cough: light is more.There is statistical significance in the degree of cough, the amount and color of sputum compared with qifen group. The frequency occurrence of the light degree of cough in positive group is higher than the negative. The amount of sputum in positive group is lower than the negative.The white color is dominated. In terms of thick and thin phlegm there was no statistical significance compared with negative group.②Other symptoms:poor food intaking,asthenia,tired,feverish palms and soles,upset and insomnia,dry stool.5. Conclusion5.1 Entirety conditionThe entirety condition of antibiotic-resistant bacillus pneumonia in the elderly was severe, had a long course and a high death rate and combined high basic illness.5.2 Characteristic of symptom-complex5.2.1 In the stage of weifen syndrome, the symptoms were not typical.The mainsym-ptoms were cough, white sputunm, and pant. Many patients had no the course of the weifen.5.2.2 In the stage of qifen syndrome, the main symptoms were fever(medium and high fever),cough, yellow and phlegm sputum, pant and distress that always influence digestive system. It is asthenia in origin and sthenia in superficiality, what is deficiency in origin is deficiency of qi an yin, what is sthenia in superficiality is that sputum, heat and stagnation.5.2.3 In the stage of yingfen syndrome, the main symptoms were nigt fever and abnormal in spirit while the symptoms of cough and sputum decresed.5.2.4 In the stage of faint and depletion syndrome,it is easier to faint. The death rate is high and the illness is dangerous.5.2.5 In the stage of deficient of vital QI leading to lingering of pathogen syndrome, the symptoms such as cough and sputum decrease, the symptoms such as dry cough without sputum or fewer white thick phlegm remained.Most will show deficiency of both vital energy and yin.5.3 The rule of transmissionThe weifen syndrome of antibiotic-resistant bacillus pneumonia in the elderly was not typical. It easily transfers to yingfen, and take an onset of faint and depletion syndrome. |
