Objective:As in the early detection of cancer and recurrence monitoring , eikonic examination and blood screening have still a considerable degree of blind spots and misunderstandings. The multiple ears information instruments of Chinese medicine have some shortcomings in the objective, quantify, standardization. We hope that this study can explore a unique new method in the early detection of cancer and recurrence monitoring。Methods:The case group: had examined 114 patients with the diagnostic malignant tumor but had no significant clinical manifestation in The First clinical hospital of china medical university (institute affiliated hospital), Liaoning cancer hospital (hospital of province ), BenXi steel company general hospital (hospital of city), BenXi tuberculosis hospital, male 66, female 48, the age from 16 to 75, by E examination (research and produce by YaoHeng international investment limited company).The number of control group is 420 which include 154 males and 266 females. Concurrence double-blind controlled four-tables analysis compared with gold standard (pathology diagnostic or cytology diagnostic), and evaluating objectively the value of E examination for diagnostic malignant tumor, include accuracy and reliability, such as the confidence interval of sensitivity and specificity.We made monitoring statistic with"E examination"for general people.The subjects 333 (male 138, female 195) are randomized general people from the center of medical test in Taipei, the age from 7 to 91.The method of dealing with data:To valuate the E examination accuracy, we computed Coincidence rate, Prevalence rate, Sensitivity, Specificity, Positive predictive value, Negative Predictive Value, Positive likelihood ratio, Negative likelihood ratio, and the confidence interval of Sensitivity and Specificity in diagnostic test. The criterion of positive findings: after the cross detection analysis of hands and ears, if the abnormal index of TA≥9/12 (highest is 12/12), this can be classified into positive; if the abnormal index of TA≤8/12 (lowest is 1/12) , this can be classified into negative.Based on the examined findings, we made continuous 5 years follow-up with false positive of E examination. We can record whether or not the population developed malignant tumor and diagnostic method and so on.Results:The E examination evaluation of accuracy: The coincidence rate is 62.4%, the prevalence rate is 21.4%, the sensitivity is 91.2%, the specificity is 54.5%, the positive predictive value is 35.3%, the negative predictive value is 95.8%, the positive likelihood ratio is 2.0, the negative likelihood ratio is 0.16 respectively. The 95% CI of sensitivity and specificity of E examination in evaluation of precision is 86.0%~96.4%, 49.7%~59.3% respectively.The Coincidence rate, Prevalence rate, Sensitivity, Specificity, Positive predictive value, Negative predictive value, Positive likelihood ration, Negative likelihood ratio of E examination in evaluation of accuracy in random population (high-risk group) is respectively 89.8%, 31.5%, 98.1%, 86.0%, 76.3%, 99.0%, 7.0, 0.02. The 95% CI of Sensitivity and Specificity of E examination in evaluation of precision in random population is 95.5%~100% and 81.5%~90.5%.The follow-up findings of false positive: 194 subjects were false positive of E examination by gold standard, 3 subjects were diagnosed malignant tumor by clinical pathology after 3 years follow-up, a subject was breast cancer after 1 year, a subject was breast cancer after 3 years, a subject was cervix cancer after 3 years.Conclusions:The Sensitivity of E determination is high, especially in high-risk group, the Positive likelihood ratio is 7.0 and its precision is much better. The E determination is physical diagnosis methods, convenient and easy to use, with no virulence, no damage, no pain, no sequel, low cost, has relative preponderance in early diagnosis and recurrence monitoring of malignant tumors.
