1. Significance Of Detecting with Early Stage Subclinicalvascular Alterations in Different Blood Pressure LevelsObjective To investigate subclincal vascular alterations in artery compliance and carotid artery IMT(intima-media thickness) among subjects with different blood pressure levels.Methods Subjects with normal blood pressure (n=69) , with high-normal BP (n=42), with grade 1 hypertension (n=87) and with grade 2 and above hypertension (n=38) were enrolled in this investigation. Arterial compliance (C1-large artery and C2-small artery) was measured by HDI-2020 and common carotid artery IMT (CCAIMT) was determined by B-mode ultrasound.Results Covariance analysis showed that Ciand C2 were decreased with the elevation of BP level. Significant differences of C1 and C2 were found between groups (p<0.05). Patients with grade 1, 2 and above hypertension had significant higher CCA IMT than subjects with normal and high-normal BP level (p<0.01). Compared with normal BP group, no higher IMT was detected in high-normal BP group. Chi-square analysis revealed the clinical decrease of C1 were present in 0%, 1.4%, and 2.4 % of normal, high-normal BP and hypertensive subjects respectively, but these differences did not reach statistical significance(p>0.05). The prevalence of clinical decrease of C2 in normal, high-normal BP, and hypertensive subjects was 23.3 %, 42.9%, and 77.9 % respectively (P<0.05).Conclusions The decrease of C2 can be demonstrated before the elevation of blood pressure, the decrease of C\ and the increase of CCAIMT, and may be as earlier and more sensitive index than the two others in reflecting subclincal vascular alterations among subjects with different blood pressure levels.2 Association between metabolic syndrome and arterial compliance and the carotid artery intima-media thicknessObjective To investicate the association between metabolic syndrome (MS) and arterial compliance, common carotid artery intima-media thickness(CCAIMT).Methods According to the definition of MS proposed by ATPIII in 2001, totally 211subjetcs were enrolled and divided into four groups by the clusting number of its componets such as control group, group 1 with one component , group 2 with two component and group 3 with three and more . Arterial compliance (C1-large artery , C2-small artery )were measured by HDI DO-2020. CCAIMT was determined by HP5500ultrasound.Results 1 Compared with control group , C1 and C2 were dereased and CCAIMT increased((P<0.010.05) and with the clusting of components of MS, the alterations of C1, C2 and CCAIMT were gradually marked . Compared with the alterations of C1 and CCAIMT , Reduced C2 was observed earlier and found in subjects with only one component (P <0.05). 2 Patients with clinical decrease of C2 had significant greater HOMA— IR(insulin resistance ) than those without it (P <0.01) . The differenc was also observed in patients with alterations of C1 or CCAIMT, but reached no significance (P>0.05 ) .3 Spearman rank correlation analysis revealed HOMA —IR, CCAIMT were positively correlated with the rank of components of MS (P <0.01 respectively ) , C1 and C2 were negatively correlated (P <0.05 respectively).Conclusin 1 Reduced artery compliance and greater CCAIMT may be assoC1ated with MS and the clusting of its components and may proceed the currence of MS. 2 The decrease of C2 may be as a sensitve index to refect vascular diseases with early stage, endothelial dysfunction caused by IR.3.Relationship between aldosterone synthase (CYP11B2) gene - 344C/ T polymorphism and small artery compliance (C2)Objective To investigate the relationship of aldosterone synthase (CYP11B2) gene - 344C/ T polymorphism and small artery compliance(COMethods (1) C2 was measured with CVProfilor DO-2020 among 224subjects enrolled in the study , including 123 abnormal C2 subjects and101 controls (2) .
