Objective: To compare the recent effect of Trans-anal one-stage Soave pull-through with Duhamel style for Hirschsprung's disease (HD) in children, and to understand the correlative factor with the result.Method: Trans-anal one-stage Soave pull-through procedures for HD were performed in 47 children, then compared this group to that of Duhamel procedure. Some factors such as operation time, fasting time, bleeding, staying in hospital are analysis. Blood sample were taken to monitor the change of status of T-lymphocyte sub-population in peripheral blood of children. They were follow up in regular time. Anus function were evaluated and data were analysis in SPSS.Result: The operative duration of trans-anal one-stage Soave pull-through for HD was longer than that of Duhamel style, but other items such as fasting time, the time of intravenous fluid and antibiotics, hospital stays et al. were superior to that of Duhamel. Anus function and complication for two groups were about the same. The immune status of HD children performed by Trans-anal one-stage Soave pull-through procedures was less restrained than that of control part. Enteritis is aninfluence factor to prognosis. A relative cut off is found in the 60th days after operation. The patients suffered from longer lengths of aganglionic distal bowel recover slower than that of other types. The patients suffered from Intestinal neuronal dysplasia (IND) have a longer recover time.Conclusion Trans-anal one-stage Soave pull-through procedure for HD is better than that of Duhamel style in infant. The advantage of this procedure is minimal trauma and quick recover. It is save and effective. Enteritis is a relative factors of influencing prognosis. The 60th days after operation is a relative cut off. Enteritis is an influence factor to prognosis. The patients suffered from longer lengths of aganglionic distal bowel recover slower than that of other types.
