Objective:To analyse the superiority of the fenestration lumbar discectomy by comparing the results of three different surgical approaches for treatment of lumbar disc herniation.Methods:Three-hundred and seventy patients undergone operative treatment for lumbar disc herniation were reviewed. Group A consisted of 300 cases were done through fenestation. Group B of 40 cases were done through semi-laminectomy, and group C of 30 cases through total-laminectomy. All cases were fellowed-up from three monthes to two years. There was no significant difference between three groups in sex, age, symptoms and signs before operation. Length of cuts, the blood loss, the operation time, the post-operation hospital stay time, complications and the post-operative results were surveyed.Results:There were significant difference in all indices between three groups. The excellent and good rate of 97%, 84%, 17% respectively in the groups A, B, C. The fenestation lumbar discectomy had the best outcome and the least complications. Whreas the total-laminectomy discectomy bore the worst outcome and the frequently encountered complications such as intability dura complications, and etcs.Conclusions:The fenestation lumbar discectomy shows less trauma and faster recovery. It is safe and effective in the treatment of lumber disc heniation.
