We present a case of paraneoplastic pemphigus associated with Castleman's disease. Our patient is a 64 years old woman. There are diffused maculopapules in the trunk and extremities and digestive tract symptoms. Gastroscope indicated that there are diffuse erosions, ulcers and bleeding in the esophagus, stomach and duodenum. The clinicopathologic diagnosis of paraneoplastic pemphigus was confirmed by biopsy of tumor for Castleman's disease, direct immunofluorescence staining of linear C3 deposition in the cutaneous basal layer and indirect immunofluorescence staining of IgG against desmosome between keratinocytes.
