| On the basis of treatment review on fatty liver with TCM, we summed up the tutor' s clinical experience in fatty liver treatment, combined modern medicine achievement on fatty liver, and thus presented our view on the liver and spleen. We selected fatty liver belonged to phlegm stagnancy and blood stasis as the subject, established the method of strengthening spleen, resolving phlegm and promoting blood circulation, prescribed Modified Bupleurum and Peony Plus Two Vintage Herbs Decoction. It was investigated the effect of strengthening spleen, resolving phlegm and promoting blood circulation on fatty liver belonged to phlegm stagnancy and blood stasis. The blood biochemistry, Hpid and type B ultrasound were observed in treatment before and after.In theories research, we explored the etiology and pathogenesis and therapeutical principle based on the theory of western and traditional Chinese medicine, emphatically discussed the relationship of the liver and spleen to fatty liver, and the importance and feasibility to treat fatty liver from the liver and spleen. Furthermore we established the principle of designing a formula on strengthening spleen, resolving phlegm and promoting blood circulation (i.e. Modified Bupleurum and peony Plus Two Vintage Herbs Decoction ) .In clinical research, it was established strict diagnosis criteria, inclusion criteria, exclusion criteria and effective evaluation criteria firstly. 50 qualified cases were divided into two groups at random. 26 cases in treatment group were treated with Modified Bupleurum and Peony Plus Two Vintage Herbs Decoction and diet control. Other 24 cases in control group were treated with diet control. The therapeutic effect was observed in two groups after treatment single-blindedly. The course of treatment was three months. The results showed that Modified Bupleurum and Peony Plus Two Vintage Herbs Decoction had the effect decreasing serum lipid and glulamic-pyruvic transaminase (OPT) . The level of serum GPT in treatment group decreased from 51.88?18.68 U/L before treatment to 34.19?10.39U/L after treatment. Mean of total cholesterol( TC) and triglyceride ( TG ) also decreased from 232.04?33.36mg/dl - 263.73 ?124.17mg/dl before treatment to 188.811 11.41mg/dl* 148.35?50.15mg/dl after treatment respectively. There was significantly difference in T-C and T-G in treatment before and after ( P < 0.01). The descended range in GPT - T-C and T-G in treatment group was significantly higher than those in control group (P <0.05 ' P<0.01) . Compared the therapeutic effect, total effective rate and marked effective rate in treatment group were 76.92% and 46.15% respectively ?Those in control group were 50% and 0% respectively. The therapeutic effect in former was better than that in latter ' with significantly difference between two groups (P<0.05 ?P<0.01) . It is concluded that Modified Bupleurum and Peony Plus Two Vintage Herbs Decoction can decrease the levels of serum GPT ?T-C and T-G and improve fatty degeneration in the liver observed by ultrasound. Its effect is obviously better than that in control group treated with diet control.In summary, therapy of strengthening spleen, resolving phlegm and promoting blood circulation on fatty liver belonged to phlegm stagnancy and blood stasis can obviously decrease the levels of serum GPT, T-C and T-G. It can not only promote the normalization of degenerated hepa- tocytes, and liver cells regeneration ?but also can prevent steatosis and fibroplasias, relieve fatty infiltration into the liver and therosclerosis, which are possibly related to its prevention lipid from stagnating in serum or infiltrating into inner wall of the vessel. |
