| Objective①To study the clinical manifestation and therapy of psoriasis vulgaris;②To observe the efficacy and satety of Pulian cream when it was used to treat blood heat syndrome of psoriasis.Contents①Collected the clinical cases of psoriasis from 1999 to 2009(from Guang Anmen hospital). according to The selective standards,A total of 482 psoriasis vulgaris patients were collected and studied. Then we analyze the clinic characteristics and the treatments.③We designed the clinical trials by means of forward-looking and random and control allocation.50 cases of patients was included in this study,48 patients completed the survey,26 in group A and 22 in group B.4 weeks was considered as a course of treatment, while we observe the symptom in the clinical once 2 weeks. Using the internationally scores of Psoriasis Area and Severity Index(PASI) to measure the skin lesions, the scores of erythema, scale, the degree of infiltration and lesion size were assessed at baseline, the 2 weeks,4 weeks, in treating time, for each time point, the changes of the lesions were compared. At the same time,we use the TCM symptom grading to observe the cream's role in easing TCM symptom and the TCM symptom grading's feasibility,we are trying to explore more practical evaluation of TCM syndrome. In addition, the safety of the treatment was evaluated, we made close observation about the adverse events with the patients, safety inspection (including:blood and urine routine examination, liver function test, renal function test) done in all patients at the baseline and the finish time.Result①At last we collected 482 cases,The ratio of male and female was 1.8:1,the average onset age was 32.54±16.46, The average length of hospital stay was 37.93±37.77, The exacerbation season was winter and spring. the most onset case was upper respiratory infection for the first time,the reason of exacerbation were upper respiratory infection and bad habits. some patients had family(19.2%) history,81.7% patients got pruritus. About 51.5% patients' tongues were described as red tongue,44.8% were dark,with 23.7% Baini and 51.87% Huangni fur,and 23.7% patients has slippery pulses and 46.1% has String slide pulses. the Top two patterns were the syndrome of blood heat(45%), stagnated heat(35.3%). Drug used most was antihistaminic. Chinese ointments were used most for external use. the medicine used for intravenous therapy was mainly Qingkailing.Xuanshen,Zicao,tufuling were used most frequently with blood heat syndromes;Baiying,Shemei,Daqingye were also used most frequengtly with stagnated heat syndromes,which were all heat-clearing and detoxication herbs.②The PASI score decreased after the treatment in both groups,which can prove that both cream can improve the symptoms of psoriasis. The group A's PASI score significantly decreased than that of the group B,which indicates that the group A's treatment effect is better than that of the group B. the rate of cure and excellently effective is 44% in group A and 15% in group B. There were significant differences between the two groups.these indicate that the group A can improves the symptoms of psoriasis better. There is no severe side effect observed.The patients'TCM symptoms of tongue, pulse, itching, upset and irritability were notably alleviated in all of the 2 groups. Considering of tongue, itching, upset and irritability, there were statistical significances between the group A and the group B,these all proves that two groups both can alleviate TCM clinical symptoms,but the group A's effect is better,these also indicates that TCM symptoms grading can analysis treatment effect in another aspect.Conclusion①There are some characteristics of the psoriasis vulgaris in our hospital②In summary,it is concluded that the Cream of group A is effective and safety in treating blood heat syndrome of psoriasis.Furthermore,the TCM symptoms grading can analysis treatment effect in another aspect. |
