| Latent glomerulo nephritis(LGN),also termed asymptomatic hematuria and(or) proteinuria, is slight to moderate hematuria (MH or GH) and(or) proteinuria, not accompanying oedema, hypertension and of the primary glomerular disease induced by renal dysfunctions. In recent years, LGN trends an increase in incidence. However, no active treatment has not been emphasized for the reason that the disease process mostly fails to affect other organs with no obvious symptoms being observed in normal days, the course of disease is long enough even form 20 to 30 years without complete cure, and that past studies have suggested this disease a stable condition and a favorable prognosis. The research progress in. EBM(Evidence-based Medicine) and the recent up-to-date discoveries in studies on LGN suggested that LGN patients have pathologic damages in kidney, and fibrosis has begin to develop. Treatment, if not timely carried out, part of patients (about 20%-30%) will suffer renal function decline, renal insufficiency and eventually uremia. Therefore, it is of great importance to look for an effective means of early intervention.It has been confirmed by clinical trials that traditional chinese medicine has to some extent, has its advantages in prevention and treatment of LGN. Based on the major technical subjects of the tenth Five-year Plan, the present research combined Lvrenhe's clinical experience in treating CKD, practiced dialectical treatment depending on "three types and four symptoms" therapy proposed by Lv renhe,and used the theory of yishen buxu huayu sanjie for the treatment method. And adopted TCMs such as raw astragalus root, rehmannia dride rhizome, glossy privet fruit, ecliptae herbal, gordon euryale seed, chrokee rose fruit, hairyvein agrimonia herb, Cyathula Root,Sanchi powder as the principal agents, and made combinations as well as modifications according to symptoms to perform intervention treatment on patients suffering LGN, thus to explore the efficacy and mechanism of the treatment for LGN.The research is a randomized controlled trial. And in this study,92 LGN patients were chosen for an 8-week observation trial. The observed therapeutic evaluation index include:TCM symptoms score, red blood cell of urine,24-hr urine protein, and GFR etc. Safety observation index include:blood routine test, Liver and kidney function test etc. Statistic Application, Use statistical software:spss17.0 service pack to make comparisons in/among groups on these indexes before or after treatment, and methods such as t-test, x2 test, rank-sum test and variance analysis.Results showed that:(1)LGN of haematuria type, in treatment group,29 cases were studied, clinical control 6 cases, marked effective 8 cases; effective 8 cases; nvalid 7 cases; total 75.9%;in control group,22 cases were studied, clinical control 1 cases,marked effective 3 cases.effective 2 cases, invalid 16 cases, total 27.3%; (2)LGN of urine protein type, in treatment group,5 cases were studied, clinical control 2 cases; marked effective 1 cases, effective 2 cases,total 100%; in control group,7 cases were studied, effective 2 cases, invalid 5 cases,total 28.6%;(3) Haematuria and urine protein type(mixed type),in treatment group,16 cases were studied, clinical control 2 cases, marked effective 5 cases, effective 4 cases, invalid 5 cases, total 68.75%;in control group 13 cases.clinical control 1 case; marked effective 2 cases,effective 1 cases,invalid 9 cases, total 30.77%.From the comparison between treatment group and control group, the total effective rate of treatment group had a obvious advantage over that of the control group (P<0.05 for all) with significant disparity in statistics.This research proved that the theory of yishen buxu huayu sanjie did a good job in improving the clinical symptoms of LGN, decreasing red blood cell counts of urine as well as 24-hr urine protein, and on romoting GFR and controlling development of LGN. The methods and the TCMs used are safe, and it is an alternative protocol for early intervention treatment of LGN using TCMs. |
