| Objective:Chronic urticaria is a common and frequently occurring clinical dermatology disea se. Western medicine believe this is due to small blood vessel of skin and mucous membrane dilation and permeability increased in response to the emergence of a li mitation edema, the pathological mechanisms are not yet fully elucidated, but most patients have been confirmed with type l allergy related. Currently, Western medicine is based mainly on symptomatic treatment, first-line drugs are antihistamine, but its efficacy in chronic urticaria is very poor. In addition, the adrenal corticosteroids, im munosuppressive agents as well as desensitization therapy, also are commonly used i n Western medicine treatment of chronic urticaria, but all have some adverse reactio n or risk factors. TCM in the treatment of chronic urticaria has advantages of definit e efficacy, less side effects, lower recurrence rate. Scholars of modern Chinese medici ne's understanding of the hives are coincident, that the incidence is mostly associate d with Feng Xie, and may carry cold, heat and dampness. However, among the clinic al, treatment based on conventional ideas can't take effect on all patients. Therefore, this study is going to complete the ideas of present TCM treatment starts from Tra ditional Chinese Time Medicine, extracts the relevant treatment theory from the Chin ese classic "Suwen·Zang qi fa shi lun" and "Synopsis of Prescriptions from the Golde n Chamber", designs out a set of diagnosis and treatment methods use of the patho genie Law of chronic urticaria. Therefore, through this experiment, to object clinical e fficacy of the treatment of chronic urticaria by using the the time medical theories i n " Suwen-Zang qi fa shi lun ", and providing theory for this method's wide range o f applications in clinical.Method:All patients in this study came from the patients who treated in Dermatology clin ic of Guang An Men Hospital of China Academy of Chinese Medical Science from Ju ne 2010 to March 2011. In accordance with the diagnostic criteria of "The guide dia gnosis and treatment of chronic urticaria·2007 version" and the"Suwen·Zang qi fa shi lun" five Zang disease Hui Jing Shen rhythm, we selected 75 cases of patients who met the inclusion criteria, and grouped them according time theory standards. The self-recipe Chinese herbs in the treatment are reference to pharmacologic action the ory of generation-restriction among five elements in "Synopsis of Prescriptions from t he Golden Chamber". Using before and after self-control method, patients in each gr oup were given correspond orally decoction, one dose a day, and continued for 4 w eeks, observed the symptoms and lesions improvement, in order to determine the effect of integration.Results:The total 75 patients in this study,17 cases were cured,19 cases were markedly effective,26 cases were effective,13 cases were ineffective. The total effective rate was 82.67%. In which 11 cases of hepatic asthenia group,2 cases cured,3 cases m arkedly effected,4 cases effected,2 cases were ineffective, total effective rate was 8 1.82%; in cardiac deficiency group 24 cases,6 cases were cured,8 cases were marke dly effective,5 cases were effective,5 cases were ineffective, total effective rate was 79.17%; in splenic asthenia group 13 cases,3 cases were cured,2 cases were mark edly effective,5 cases were effective and 3 cases were ineffective, the total effective rate was 76.92%; in pulmonasthenia group 9 cases,2 cases were cured 4 cases wer e markedly effective,3 patients were effective,0 case was ineffective, the total effec tive rate was 100%; in renal deficiency group 18 cases,4 cases were cured,2 cases were markedly effective,9 cases were effective,3 cases were ineffective,the total ef fective rate was 83.33%. The results showed that the treatment of the five Zang-org ans group were effective, by comparison, each Zang-organ group treatment of chroni c urticaria has no significant difference.Conclusion:Through the observation of the subject, confirmed the use of time medical theor y in "Suwen·Zang qi fa shi lun" can effectively guide the clinical treatment of chronic urticaria. The clinical application of this theory can make up and enrich the current TCM treatment of chronic urticaria, it is worth to further study. |
