| Shang Han Za Bing Lun (Treatise on Cold Diseases and Miscellaneous Diseases) was written by Zhang Zhongjing in last years of Eastern Han Dynasty about AD 205 years. The book integrates the thought Zhang Zongjing with some famous medical books that can be seen at that time. It is the first medical book that contains principles, methods, formulas and medicines. The book contains 269 formulas and 214 Chinese medicines. Because the rigorous compatibility and outstanding curative effect, the formulas have great influence on the development of Chinese medicine and are called Classical Formula. Every formula gives clear indication of doses, frequency and time of taking medicine, even the way of decoction. However, due to the evolution of metrology, drug dosage forms, the method of taking medicine, the clinical dosages of Classical Formula are not according to the book. Dosage is the guarantee of curative effect and security. But the chaotic use of dosages is according to clinical experience. So we need to set a unified dosage standard. This paper mainly studies the dosages application of some famous ancient doctor. So I choice the following six frequently-used Classical Formula:Decoction for Regulating the Middle, Decoction for Cold Limbs, Chinese Angelica Decoction for Restoring Yang, Decoction of Pueraria, Scutellaria and Coptis, Pinellia Heart-Purging Decoction. The evolution of dosages application will help doctors to use Classical Formula and help us to know the original dosages of Classical Formula. The mainly research methods, contents and conclusions are as follows:1. Summarizes the change of metrological system in ancient time, analysis the influence of the change on dosages.The change of metrological system is an important factor cause the chaotic dosages application. Many doctors give different answers according to the evolution. In East Han dynasty time, a catty is approximately 250 grams, following the Qin Dynasty system. The weight of catty is increase sharply in Wei Jin, Southern and Northern Dynasties period. In Sui Dynasty, a catty is approximately 250 grams and gradually steady. But there were two metrological systems in this time. From Tang Dynasty to Qing Dynasty, the weight of catty is slightly declining. According to the analysis of the change, the dosages of Classical Formulas in East Han Dynasty are about five times of reference dosages given by current textbook. 2. Summarize the current way of converting the dosages of Classical Formulas.Current way to convert the Classical Formula dosages mainly from four following perspectives:metrology, clinical experience, Shennong scales, actual measurement of Chinese medicines and water. The conclusion is not consistent.3. Summarize the current understanding of the dosages.Currently, the small dosages supported by of textbooks, Japan physicians, the large dosages supported by some doctors from the perspective of curative effect and experience. Also some researchers from the perspective of method of taking medicine, fresh medicine, decocting method, drug dosage forms, think that the large dosages of used in the time of Zhang Zhongjing has inevitable causes.4. Looking for regulars from medical books of Song to Qing Dynasty.Use the way of evidence-based medicine and statistical analysis. The dosages have no obvious change before Song Dynasty. The Classical Formula dosage forms are changed to decocting powder, dosages is smaller in Song, Jin and Yuan Dynasty. In Ming and Qing dynasty time, dosage forms are recovered to herb slice but following the dosage Song, Jin and Yuan Dynasty. So the decocting power way in Song Dynasty is an important cause of the Classical Formula dosages decrease sharply. |
