Depression is a common mental disorders, it has significant and complex adverse impact in the cognitive function, physiological functions, complications, and metabolic regulation and treatment of patients with diabetes. Depression and type 2 diabetes are interacting each other.The diabetes patients' blood glucose easily fluctuate if combining depression, the main responsibility of depression is liver, once emotion is disharmony, it will firstly affects liver's smoothing and dispersion. "The disease of diabetes, is nearly principal responsibility with liver not lung." Long-time liver disharmony, easily lighting a fire, damaging yin substance, this is one of the main causes.Research topics:The related factors of type 2 diabetic patients with depression and Traditional Chinese Medicine (TCM) syndromes distribution Research.Objective:Type 2 diabetes is a glucose and lipid metabolism disorder disease, its damage main on chronic complications. These complications of diabetes have become an important factor of disabled or death. Depression is major and important related complications, increasing the risk of deterioration. The research attempts to explore the related factors of type 2 diabetic patients with depression, summarize the TCM syndromes, providing references for the syndrome differentiation and treatment of patients with type 2 diabetes.Methods:We analyzed 120 cases of type 2 diabetic patients from TCM Diabetes department of the China-Japan Friendship Hospital during October 2010 to March 2011. Depressive status was assessed by the Zung Self-rating Depression Scale (SDS) in 120 type 2 diabetes mellitus patients, and respectively given clinical observation including gender, age, marriage state, family history, past medical history, etc., and measure blood glucose, lipids and other biochemical criterion. Divide the patients into depression group and the non-depression group according to the SDS, study the related factors which influence type 2 diabetes mellitus combining depression; we divide 120 patients into five kinds of syndrome—Stagnation of heat in liver and stomach, liver Qi Stagnation, Liver depression and spleen deficiency, deficiency of liver- kidney Yin, deficiency of kidney essence, and summarize the appearing probability of every syndrome distribution rule.Results:(1) gender, age, education level, occupation, marriage state, and diabetes duration, family history, complications, body mass index, history of hypertension, smoking history, sleep time, blood lipids, fat liver, HbAlc, HS-CRP, blood uric acid are significantly related with type 2 diabetes combining depression; (2) liver function, drinking history, coronary heart disease history have no relation with type 2 diabetes combining depression; (3) every syndrome in depression group are significant different, among them,Liver depression and spleen deficiency is given the highest mark of SDS, liver Qi Stagnation rating second.Conclusions:(1) high-risk groups type 2 diabetes with depression, a.female b. middle-aged c.higher education level, engaged in the white-collar worker d. unmarried or widowed e. diabetes family history, long-duration of diabetes and complications f. history of hypertension g. smoking history h. body-mass index exceeding bid. (2) The clinical doctors should take attention to those patients who are middle-aged women, especially in single, higher education level in patients with type 2 diabetes, SDS scale should be complemented to the medical records, meanwhile early give evaluation of psychological counseling. (3) Liver Stagnation plays an important role in type 2 diabetes mellitus with depression, and the two syndromes of liver Qi Stagnation, Liver depression and spleen deficiency are more in the research. Traditional Chinese medicine has shown advantages to the depression patients in improving depression rating and enhancing the quality of life, especially in the application of smoothing the liver and reinforcing the spleen have accumulated certain experience, in order to achieve the purpose of prevention and cure. (4) under the guidance of doctors, type 2 diabetes should strengthen their own management, reasonable diet, control weight,, quit smoking, limit alcohol, assure enough sleep, through their own efforts to eliminate the risk part factors of combining depression; Check HbAlc, blood lipids and HS-CRP, blood uric acid, liver type-b ultrasonic regularly, the doctor guide how to drop the reversible factors to the standard, reducing depression rating and the risk of type 2 diabetes with depression.
